Registered Nurse – Case Manager
Work Location: San Diego, CA

InGenesis has an exciting opportunity for an RN / Case Manager to work with our client located near San Diego, CA.
In this role, you will perform a blended function of utilization management (UM) and case management (CM) activities.

Job Duties/Responsibilities/Functions (including but not limited to)
• Educate subscribers via phone on resources available for their healthcare needs.
• Assist patients who suffer from both acute and chronic illness.
• Perform effective discharge planning.
• Collaborate with other professionals involved in the continuum of care.
• May provide case management to member segments with chronic or catastrophic illness.
• Perform other duties as necessary or assigned.

Minimum Qualifications
• Active Registered Nurse (RN) license.
• 3+ years’ nursing experience.
• Discharge planning experience.
• Acute care or oncology experience, a plus.
