Job Description
Member Care Coordinator
Work Location: Belleville, IL

InGenesis is currently seeking a Member Care Coordinator to conduct home health assessments and work with identified members to educate and engage them in their personal health plans. In this role, you will respond to inquiries from members and supporting the clinicians in the Medical Management department with their provider and member activities.

Job Duties/Responsibilities
• Perform home health assessments and system updates.
• Encourage compliance with care plans and alert case managers when issues arise.
• Inform and educate members on their program (may use supplied scripts).
• Encourage member usage of programs.
• Arrange appointments and additional member services (e.g., transportation).
• Generate appropriate correspondence and send manually, electronically, or telephonically.
• Conduct check-ins with members to review individual care plan goals.
• Maintain production requirements based on established business needs.
• Perform non-clinical functions necessary to generate, manage, and close a case.
• Receive, analyze, conduct research, and respond to telephone or written inquiries.
• Perform data entry function to update customer or provider information.
• Perform other duties as assigned.

Minimum Qualifications
Education/Experience
• Bachelor’s degree in Social Work or Psychology; OR
• Licensed Professional Nurse / LPN with one (1) year experience in managed care systems; OR
• Registered Nurse / RN license; OR
• 3+ years care coordination for a state managed or waiver program; OR
• 3+ years managed care systems experience.

Other Requirements
• Knowledge of medical terminology.
• Experience analyzing member needs.
• PC proficiency including Microsoft Office applications.
• Customer service skills.
• Effective communication skills (written/verbal), including coaching skills and motivational interviewing.
• Current state driver license, reliable transportation, and applicable insurance.
• Ability and willingness to travel.
