InGenesis is currently seeking a Registered Nurse / RN Case Manager to work with our client located in O'Fallon, IL. In this role, you will participate in all phases of the Case Management Program (CMP) and ensure that the CMP meets established case management (CM) standards of care.

Job Duties

  • Provide nursing expertise about the CM process, including assessment, planning, implementation, coordination, and monitoring.
  • Develop and implement local strategies using inpatient, outpatient, onsite and telephonic CM.
  • Integrate CM and utilization management (UM) and integrating nursing case management with social work case management.
  • Integrate CM and utilization management (UM) and integrating nursing case management with social work case management.
  • Develop and implement tools to support case management, such as those used for patient identification and patient assessment, clinical practice guidelines, algorithms, CM software, and databases for community resources.
  • Establish mechanisms to ensure proper implementation of patient treatment plan and follow-up post discharge in ambulatory and community health care settings.
  • Monitors and evaluates the facility’s case management program, peer review program per DoD, AF and local policies and guidelines.
  • Develops, utilizes and maintains a variety of military and community resources to optimize access to services and medical care. Ensures timely and appropriate provision of services.
  • Ensures coordination of care delivery processes, to include alternate healthcare settings and the home environment, for the purposes of enhancing the patient's health and wellness, safety, productivity, and quality of life, and for providing the most beneficial, cost-effective health care.
  • Perform other duties as assigned.


Minimum Qualifications

  • Associate’s degree in Nursing or Baccalaureate Degree in Nursing. Baccalaureate Degree is preferred, but a Master’s degree in Nursing is highly desirable.
  • Graduate from a college or university accredited by National League for Nursing Accrediting Commission (NLNAC), or The Commission on Collegiate Nursing Education (CCNE).   
  • Current Healthcare Provider (HCP) Basic Life Support (BLS) certification using American Heart Association or the American Red Cross guidelines. Members required to have Advanced Life Support requirements (ACLS, PALS, NRP) must present proof of current certification(s) thru American Heart Association or the American Red Cross.
  • Computer-based training does not constitute fulfilling the training requirements.
  • Possess one of the following certifications is desired: National Board for Certification in Continuity of Care (ACCC), Commission on Rehabilitation Counselor Certification: Certified Rehabilitation Counselor (CRC), American Nurses Credentialing Center Nurse Case Manager (RN-NCM), National Academy of Certified Case Managers: Care Manager Certified (CMC). Maintain an active, valid, current, and unrestricted license (with no limitations, stipulations or pending adverse actions) to practice nursing as a registered nurse in any US state/jurisdiction.    
  • Experience: Prefer 4 years of active nursing practice, 18 months of which must be in discharge planning, clinical case management for pediatric patients and their families (may also work with adults, seniors or groups). Must have worked in nursing in the past 12 months. Home Health experience is desirable.   
  • Licensure/Registration: Current, full, active, and unrestricted license to practice as a Registered Nurse as required in the TO.
