RN - Nurse Care Manager (Behavioral Health)

Job Description

A professional mental health staff serves a growing caseload of 2,800 patients, of which greater than ninety-five percent are chronically mentally ill and suffering from major psychiatric disorders. Hopewell Center operates a Psychosocial Rehabilitation Center and a Comprehensive Psychiatric Rehabilitation Center for adults and youth. Many services are provided at schools. Emergency services, including a crisis bed and mobile outreach, are available twenty-four hours per day. A multi-disciplinary team of psychiatrists, psychologists, nurses, counselors, social workers, educators, and support staff work diligently to carry out the mission of providing comprehensive care to patients and their families.

Essential Duties and Responsibilities:

The Nurse Care Manager explains procedures and treatments to patient to gain cooperation, understanding, and alleviate apprehension in a behavioral health clinic setting. Assists w/ developing and enhancing wellness initiatives such as wellness support groups, nutrition and physical activity programs, periodic staff and consumer education classes. Promotes the use of Direct Inform with clients. Consult with Community Support staff regarding health conditions of clients, Support and education CSS staff on health coaching. Administers prescribed medications and treatments in accordance with approved nursing techniques. Maintains awareness of comfort and safety needs of patient. Observes patient, records significant conditions and reactions, and notifies supervisor or Physician of patient's condition and reaction to drugs, treatments, and significant incidents. Takes temperature, pulse, blood pressure, and other vital signs to detect deviations from normal and assess condition of patient.

Assist in developing the treatment plan for clients, and identifying healthcare goals. Collaborate with DMH state-operated facilities and general hospitals in discharge planning of HH caseload. Documents nursing history and physical assessment for assigned patients. Initiates a patient education plan according to the individualized needs of the patient, as prescribed by Physician and/or hospital policy including patient and family instruction. Participates in department or unit quality improvement activities.

Qualifications:

Missouri RN licensure, CPR certification and a minimum of 3-5 years of clinical nursing experience. Experience working in an ambulatory outpatient clinic or community health setting is strongly preferred. Qualified candidates must be capable of working in a fast-paced clinical environment, able to handle several tasks and multiple priorities. Excellent assessment and triage skills, strong interpersonal, customer service organizational and communication skills are essential, as is the ability to work effectively in a team environment. Must demonstrate cultural competence working with a diverse patient population.
