Position Overview
Care Medical Practice model is designed for member engagement of the high-risk population with an emphasis on event-driven care management leveraging care pathways and evidenced based guidelines tailored to black and brown populations. Care Management includes assessing healthcare needs, identifying problems and opportunities for improvement, implementing patient Action Plans, managing the patient care transition process, assisting patients throughout care episodes, coordinating, and facilitating care for patients with complex, chronic medical and mental health conditions, providing disease education, and promoting evidence-based healthcare services. The individual in this position works as part of an interdisciplinary team to ensure high quality outcomes for patients/families struggling with chronic disease management. It is critical that care management be done in conjunction and always with the Caregiver, including their and the member’s signoff. Conducting Caregiver assessments are also part of the Care Management process.
Key Responsibilities
- Conducting well-woman exams
- Reviewing preventive health needs
- Educating and prescribing available contraception
- Inserting long-acting reversible contraception such as Intrauterine devices and Implanted devices
- Educating and counseling on unexpected pregnancies
- Ordering and interpreting blood and imaging tests
- Addressing infertility concerns
- Sexually transmitted infection diagnosis, treatment, and education
- Managing women through the perinatal period, such as confirming/dating pregnancy; educating on pregnancy health; performing ultrasounds; and monitoring fetal activity
- Menopause education and counseling
- Intimacy and sexual health counseling
- Screening for domestic violence, substance use disorder, depression, anxiety, and high-risk behaviors
- Diagnosing female-related disease processes
- Performing primary care procedures like pap smears and endometrial biopsies
- May work alongside OBGYNs as to provide prenatal care
- Perform comprehensive evaluations including history and physical exams for risk adjustment and other regulatory assessments
- Order pertinent diagnostic laboratory and radiology testing for the medical diagnoses or presenting symptoms; able to work within an environment of limited resources and therefore use diagnostic tests judiciously and appropriately Create and implement a medical plan of care
- Provides care coordination and preventative care services for geographical as well as virtual targets who are stratified by risk
- Provide post-discharge coordination to reduce hospital readmission rates and emergency room utilization
- Perform face-to-face in-person visits in clinic and in-home settings
- Additionally, perform face-to-face synchronous video communications using Tele-Health platforms based on business needs and leadership direction
- Prescribe medications as appropriate
- Perform timely documentation in an electronic medical record computer system or other method of documentation
- Engage in practices constituting the practice of medicine in collaboration with and under the medical direction and supervision of a licensed physician to the degree required by state laws.
- Collaboration: The Women’s and Maternal Health Nurse Practitioner will work closely with other clinical and business divisions to learn their needs, internalize their knowledge, and define solutions to achieve the clinical objectives of Care Medical Practice’s model of care.
- Knowledge: The Nurse Practitioner will provide clinical subject matter expertise, seek specialty consultation as appropriate, and is responsible for knowing when a patient needs physician oversight and when the patient's medical condition may be beyond the scope of knowledge of the NP.

Required Qualifications
- Education:
- Certified and registered as a Women’s Health Nurse Practitioner (WHNP), Family Nurse Practitioner (FNP), or Certified Nurse Midwife (CNM) by the New York State Education Department (NYSED) to practice as a WHNP, FNP, or CNM in New York
- Licensed to practice in NYS.
- Graduated from a nurse practitioner education program acceptable to NYSED or certified as a nurse practitioner by a national certifying organization acceptable to NYSED.
- Completed pharmacology coursework acceptable to NYSED
- Applied for nurse practitioner certification with NYSED
- Experience:
- Previous experience as a WHNP, FNP, or CNM, 3-5 years
- Preferred experience in women’s health programs as a licensed clinician
- Experience with underserved populations facing socioeconomic barriers to healthcare
- Fluency in a language in addition to English is a plus
- Immunization and phlebotomy skills
- Communication: Excellent verbal, written communication, and presentation skills; ability to clearly articulate and present concepts and models in an accessible manner to Care Medical Practice’s team, investors, partners, and other stakeholders. Proficiency in all Microsoft Office applications.
- Relationships: Ability to build and effectively manage relationships with business leaders and external constituents.

The working environment and physical requirements of the job include:
Work is performed indoors in a setting with conditioned air and artificial light. Travel to and work in offices or other environments is required.
In this position you will need an to communicate with customers, vendors, management, and other co-workers in person and over devices, sometimes with people who are agitated. Regular use of the telephone and e-mail for communication is essential. Sitting for extended periods is common. Must be able to receive ordinary information and to prepare or inspect documents. Lifting of up to 10 lbs. occasionally may be required. Good manual dexterity for the use of common office equipment such as computer terminals, calculator, copiers, and FAX machines. Good reasoning ability is important. Able to understand and utilize management reports, memos, and other documents to conduct business.
