Position Overview
The Care Medical Practice model is designed for deep engagement with high-risk populations, emphasizing event-driven care management. We leverage care pathways and evidence-based guidelines tailored specifically to underserved populations.We are seeking a versatile and compassionate Family Nurse Practitioner (FNP) who specializes in Women’s Health but possesses the clinical breadth to address primary care concerns. The ideal candidate will serve as a clinical anchor for our members, providing expert gynecological and maternal care while simultaneously managing chronic conditions and acute primary care needs as they arise. You will work as part of an interdisciplinary team to ensure high-quality outcomes for patients and families navigating complex medical and socioeconomic landscapes.
Key Responsibilities
Women’s Health & Maternal Care
- Conduct comprehensive well-woman exams, preventive screenings, and diagnostic female-related evaluations.
- Provide full-spectrum contraceptive counseling and services, including the insertion of long-acting reversible contraception (IUDs and implants).
- Manage patients through the perinatal period, including pregnancy confirmation, dating, fetal monitoring, and ultrasound performance.
- Diagnose and treat sexually transmitted infections (STIs) and provide intimacy and sexual health counseling.
- Offer menopause education, infertility counseling, and support for unexpected pregnancies.
- Perform specialized procedures such as Pap smears and endometrial biopsies.
Primary Care & Chronic Disease Management
- Address and manage primary care concerns that arise during women’s health visits, ensuring holistic patient management.
- Perform comprehensive history and physical exams for risk adjustment and regulatory assessments.
- Order and interpret pertinent diagnostic laboratory and radiology testing judiciously.
- Create and implement individualized medical plans of care for chronic conditions (e.g., hypertension, diabetes, asthma) common within our target population.
- Provide post-discharge coordination to reduce hospital readmission rates and emergency room utilization.
Care Coordination & Clinical Operations
- Conduct face-to-face visits in both clinic and in-home settings, as well as synchronous video Tele-Health communications.
- Collaborate with OBGYNs and other specialists to provide seamless prenatal and specialty care.
- Screen for domestic violence, substance use disorders, depression, anxiety, and other social determinants of health.
- Maintain timely and accurate documentation within the Electronic Medical Record (EMR) system.
- Engage in collaborative practice under the medical direction of a licensed physician in accordance with New York State laws.
Required Qualifications
Education & Licensure
- Current registration and licensure as a Registered Professional Nurse (RN) in New York State.
- Certified and registered as a Family Nurse Practitioner (FNP) by the New York State Education Department (NYSED).
- National certification as an NP by an organization acceptable to NYSED (AANP or ANCC).
- Completion of pharmacology coursework and possession of a valid DEA registration.
Experience & Skills
- 3–5 years of experience as a Nurse Practitioner, with a strong background in both Women’s Health and Primary Care.
- Proven experience working with underserved populations facing socioeconomic barriers to healthcare.
- Proficiency in phlebotomy, immunizations, and basic office-based procedures.
- Fluency in a language in addition to English is highly preferred.
- Strong clinical judgment and the ability to identify when a patient’s condition requires physician oversight or specialty referral.

The working environment and physical requirements of the job include:
Work is performed indoors in a setting with conditioned air and artificial light. Travel to and work in offices or other environments is required.
In this position you will need an to communicate with customers, vendors, management, and other co-workers in person and over devices, sometimes with people who are agitated. Regular use of the telephone and e-mail for communication is essential. Sitting for extended periods is common. Must be able to receive ordinary information and to prepare or inspect documents. Lifting of up to 10 lbs. occasionally may be required. Good manual dexterity for the use of common office equipment such as computer terminals, calculator, copiers, and FAX machines. Good reasoning ability is important. Able to understand and utilize management reports, memos, and other documents to conduct business.
