Overview
The Pharmacy Technician will support the clinical pharmacist team by assisting with medication adherence initiatives, outreach to members, physicians, and external pharmacies, and ensuring the accurate processing of medication-related tasks. This role is integral to improving medication adherence, enhancing patient safety, and supporting the clinical pharmacist in delivering high-quality pharmacy care to our members. The Pharmacy Technician will collaborate closely with the clinical pharmacist team to ensure timely and effective interventions, accurate medication histories, and follow-up support.
Duties & Responsibilities
- Medication Adherence Support
- Conduct outreach calls to members to monitor medication adherence, remind patients about refills, and address any barriers to taking medications as prescribed.
- Provide education and support to members on the importance of medication adherence and proper medication usage.
- Assist with tracking adherence data, identifying gaps in care, and reporting outcomes to the Lead Clinical Pharmacist.
- Follow up with members to ensure compliance with prescribed medication regimens, offering reminders and addressing concerns as needed.
Collaboration with Physicians and External Pharmacists
- Reach out to physicians and external pharmacists to obtain and confirm medication histories, clarifications, and updates as part of medication reconciliation efforts.
- Facilitate communication between members, physicians, and pharmacies to resolve medication-related issues, such as drug interactions, formulary changes, or medication discrepancies.
- Assist the clinical pharmacist with any medication therapy management (MTM) tasks, including ensuring accurate medication histories and ensuring that all medication-related information is up to date.
Medication Review & Documentation
- Assist in reviewing medication records and identifying potential discrepancies or issues that require follow-up.
- Document all member interactions, medication updates, and any interventions in the electronic health record (EHR) or other pharmacy management systems.
- Ensure accurate and timely documentation of outreach efforts, medication adherence progress, and any follow-up steps required for members.
Clinical Pharmacy Support
- Provide logistical and administrative support to the clinical pharmacist team by organizing medication lists, coordinating medication-related tasks, and helping with follow-up care plans.
- Assist in coordinating referrals to specialty services or community resources for members requiring additional clinical support.
- Collaborate with the clinical pharmacist to monitor and track the impact of interventions related to medication adherence and therapy management.
Quality & Compliance
- Ensure compliance with regulatory requirements and company policies related to medication management and patient privacy (HIPAA).
- Participate in ongoing training and development related to medication management practices, clinical pharmacy services, and adherence programs.
- Assist in preparing reports on medication adherence trends, member outcomes, and operational performance as requested by the Lead Clinical Pharmacist.
Customer Service & Member Engagement
- Provide excellent customer service to members by addressing questions, resolving concerns, and providing information on their medications.
- Act as a liaison between the pharmacy team and members to ensure that patients receive timely, accurate, and effective pharmacy services.
Qualifications
- Education:
- High school diploma or equivalent required.
- Certification as a Pharmacy Technician (CPhT) required.
- Pharmacy Technician experience in a managed care, clinical, or healthcare setting preferred.
- Experience:
- Minimum of 1-2 years of experience as a pharmacy technician, preferably in a clinical or managed care environment.
- Experience with medication adherence programs or medication therapy management (MTM) is a plus.
- Familiarity with electronic health record (EHR) systems and pharmacy management software is preferred.
- Experience with outreach calls and working with external healthcare providers (e.g., physicians, pharmacists) is desirable.
- Skills & Competencies:
- Strong communication skills, both verbal and written, to interact with members, healthcare providers, and team members.
- Ability to work independently and as part of a team in a fast-paced environment.
- Detail-oriented with excellent organizational and multitasking abilities.
- Strong problem-solving skills and ability to handle medication-related inquiries effectively.
- Knowledge of medication therapy, adherence strategies, and clinical pharmacy practices.
- Compassionate and patient-centered approach to working with members.
Preferred Qualifications:
- Familiarity with managed care pharmacy operations, formulary management, and medication adherence strategies.
The working environment and physical requirements of the job include:
Work is performed indoors in a setting with air conditioning and artificial light. Travel to and work in offices or other environments is required.
In this position you will need to communicate with customers, vendors, management, and other co-workers in person and over devices, sometimes with people who are agitated. Regular use of the telephone and e-mail for communication is essential. Sitting for extended periods is common. Must be able to receive ordinary information and to prepare or inspect documents. Lifting of up to 10 lbs. occasionally may be required. Good manual dexterity for the use of common office equipment such as computer terminals, calculator, copiers, and FAX machines. Good reasoning ability is important. Able to understand and utilize management reports, memos, and other documents to conduct business.
