SUMMARY

The Family Coach/Case Manager is responsible for the assessments of need, provision of social services, and referrals to services for client-families who seek out services and programs from People’s Community Action Corporation (PCAC) and who need focused assistance and coaching. This will involve work with clients around needs related to education, employment, life skills, as well as other needs identified, within the specific parameters of the program. In addition, the Family Coach/Case Manager will actively seek to establish and maintain relationships and partnerships in the community to facilitate this work.

Director of Operations (if located at South Broadway)

Director of Emergency Services and Community Initiatives (if located at Lindell)

ESSENTIAL DUTIES AND RESPONSIBILITIES

Management may assign or occasionally require other duties, including direct services to clients and special events.

Assessment and Referrals

Perform an assessment of needs for each client-family using standard methods utilized by PCAC

Refer client-families as appropriate to internal and external services to meet assessed needs outside the purview of the Family Coach

Case Management

Develop a client-family plan to establish goals and steps to achievement

Follow up on client-family utilization of referrals as indicated by PCAC policy and funder requirements

Meet with the client-family on a periodic and regular basis to monitor progress

Secure any documents and signatures on any required forms for Family Coaching/Case Management, after determining that the client-families understand these documents

Participate in agency events and activities that further progress for client-families

Community Knowledge and Relationships

Maintain knowledge of internal and external referrals and agencies available to client-families

Meet with relevant PCAC staff, especially other Family Coaches/Case Managers, to review PCAC resources, services and programs available to client-families

If working with an assigned community or set of client-families, maintain knowledge and working relationships with agencies relevant to that community or set of client-families

Participate as appropriate and approved in community, professional, or peer organizations and opportunities

Participate as directed in PCAC organized and/or sponsored events and activities, even when beyond the immediate scope of the Family Coach program

Data Management

Ensure that all data is entered accurately and completely in the MIS database to account for work of the Family Coach/Case Manager

Ensure that all data is entered accurately and completely in the MIS database to assist PCAC in required reporting to funders and for PCAC to review agency work and performance

PERFORMANCE REVIEW

Probationary period

Annual written performance review by supervisor

Intermittent review as needed

REQUIREMENTS

Bachelor Degree in a related field, or five years of job-related experience

MSW or other advanced social work degree preferred

Demonstrated ability to assess client needs and take appropriate action/steps

Ability to research additional, new and/or novel resources to assist clients

Ability to use a complex database

Ability to work with diverse populations

Excellent oral and written communication skills, including interpersonal skills and ability to work well with others
