LPN - Nurse Care Manager

ALM Hopewell Center provides comprehensive mental health services to children and adults in the northern and central parts of the City of St. Louis and North St. Louis City. A professional mental health staff serves a growing caseload of 2,800 patients, of which greater than ninety-five percent are chronically mentally ill and suffering from major psychiatric disorders. Hopewell Center operates a Psychosocial Rehabilitation Center and a Comprehensive Psychiatric Rehabilitation Center for adults and youth. Many services are provided at schools. Emergency services, including a crisis bed and mobile outreach.
Provides general nursing care to patients in an ambulatory care setting. Nurse Care Manager will maintain a patient caseload using a holistic, person-centered approach for coordinating the healthcare needs and wellness goals of the patients.
Essential Duties and Responsibilities:
Coordination of care plan with health care team. Initiates a patient education plan according to the individualized needs of the patient, as prescribed including patient and family instruction. Assesses and diagnoses patient and family needs in order to provide quality care to assigned patients. Develops, discusses, and communicates a realistic problem list (care plan) for each patient, in collaboration with each patient/family/significant other in order to address all identified needs. Coordinates patient care with external healthcare workers, including providers, pharmacies, and community resource agencies. Provide education to patients and families that promotes lifestyle changes. Explains procedures and treatments to patient to gain cooperation, understanding, and assess any apprehension.
Tracks all required data and submits reporting as indicated by manager. Maintains up-to-date and accurate documentation of nursing care provided to ensure integration of information used by the healthcare team to ensure quality care and improve outcomes.
Possession of an LPN Missouri license from an accredited school of nursing. Three-five years of experience in working with the patients on the spectrum of health needs. Behavioral health experience is preferred.
Must be proficient in Microsoft Excel, Word, Power Point and Electronic Health Records.
