Intake Outreach Case Coordinator

SUMMARY

 

The Intake / Outreach Coordinator will greet visitors in person or by phone, explain services and assistance available from People’s Community Action Corporation (PCAC), and  complete initial client intake, including any needed referrals to emergency services or assistance and to Case Managers.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES


This description may not include all of the duties and responsibilities listed below nor do the duties and responsibilities listed below include every task that may be attached to the position.The Executive Administrator and the Direct Supervisor may assign or occasionally require other duties, including direct services to clients and special events.


Reception


  •  Serve as the initial contact for all visitors in person or by phone 
  • Provide information to visitors about PCAC, its services and its programs 
  • Provide information about eligibility for assistance

   

      Intake 

  • Complete client-family Intakes electronically or manually, and add Intakes to MIS system
  •  Receive and process income statements, job search forms, payroll verification, changes in address or household, and add to MIS or forward to caseworkers 
  • Ensure that all documents and information are gathered and filed by case identification to the correct client files 
  • Issue emergency assistance in a crisis situation, such as food or clothing 
  • Work in coordination with other site staff to direct client-families into case management as appropriate and needed 
  • Assist in preparation and distribution of timely reports 
  • Perform other clerical functions as assigned

 


 JOB REQUIREMENTS

 

  • High school diploma, high school equivalency, Associates Degree preferred
  •  Minimum three years of work related experience
  •  Ability to use a complex database
  •  Ability to work with diverse populations
  •  Ability to create a positive image as one of the first points of contact 
  • Demonstrated ability to assess client needs and take appropriate action/steps 
  • Excellent oral and written communication skills, including interpersonal skills and ability to work well with others
