Overview:

The VP, Grace at Home Markets serves as a strategic and operational leader responsible for ensuring that all Guided, Accountable, Clinical initiatives are fully implemented and executed. Reporting directly to the SVP Direct Care Delivery, this role leads operational integration, and cross-functional alignment to enable CINQCARE’s clinical programs to launch effectively, operate efficiently, and scale with discipline. The VP, Grace at Home Markets with help over-see all Care Delivery implementation and operation across the Grace at Home Markets with a focus on high quality care delivery to all family members.

The VP functions as the operational leader for Clinical initiatives across Grace At Home to ensure that every program is operationally compliant, efficient and successful in delivering high quality care and supported by structured governance . This role is instrumental in bringing organizational clarity, accountability, and execution discipline to CINQCARE’s Care Delivery operations.

Duties & Responsibilities

  • Lead care delivery efforts across all Grace at Home Markets
  • Direct the Clinical Manager Teams in All Markets to ensure oversight of day to day clinical operations
  • Develop and maintain a standardized care delivery framework outlining people, process, and technology deliverables required for clinical program execution.
  • Partner with the SVP to establish clinical workflows for efficient delivery of high quality clinical services to meet contract standards with the intention of improving patient outcomes and reduce Medical expenditures
  • Responsible for budget development and P&L
  • Develop, maintain Care Delivery KPIs and ensure all markets are operating within budgets and expected goals
  • Translate enterprise strategy into actionable operational clinical workflows and delivery with clearly defined deliverables, accountability, and timelines.
  • Partner with Data & Reporting to ensure operational metrics, progress, and performance indicators are met.
  • Present executive summaries to communicate operational status, risks, and goals to executive leadership


Qualifications

  • Education: Master’s degree in nursing or higher required.
  • Experience:
  • 8–10 years of healthcare operations or program management and leadership experience, with proven success in implementation leadership.
  • Demonstrated ability to lead complex, cross-functional initiatives across multiple markets.
  • Executive level understanding of healthcare delivery, compliance, and operational processes.
  • Exceptional organizational, communication, and stakeholder management skills.
  • Experience leading and managing large teams with multiple complex clinical programs.

The working environment and physical requirements of the job include:

In-office work is performed indoors in a traditional office setting with air conditioning, artificial light, and an open workspace.

In this position you will need to communicate with customers, vendors, management, and other co-workers in person and over devices, sometimes with people who are agitated. Regular use of the telephone and e-mail for communication is essential. Sitting for extended periods is common. Must be able to receive ordinary information and to prepare or inspect documents. Lifting of up to 10 lbs. occasionally may be required. Good manual dexterity for the use of common office equipment such as computer terminals, calculator, copiers, and FAX machines. Good reasoning ability is important. Able to understand and utilize management reports, memos, and other documents to conduct business.
