Position Overview

The Primary Care RN Coordinator reports directly to the (Director of Clinical Operations) administrator and plays a vital role in providing exceptional direct nursing care, optimizing patient outcomes, and coordinating key clinical operations within our primary care office setting. This position requires a blend of clinical expertise, strong communication skills, and a deep commitment to patient-centered care and quality improvement. The RN Coordinator will utilize their judgment, organizational skills, and evidence-based analysis to influence decisions, meet the specific requirements of Care Medical Practice PLLC, and drive performance on key quality metrics.

Key Responsibilities

  • Patient Care Delivery & Coordination:
    • Assessing, planning, and providing direct nursing care to family members based on their individual needs and presenting conditions.
    • Administering medications and treatments as prescribed by physicians, diligently monitoring and documenting patient responses.
    • Performing routine procedures such as injections, blood pressure checks, wound care, and other primary care nursing tasks.
    • Monitoring and accurately recording patients' vital signs, symptoms, and progress, updating care plans accordingly to ensure optimal outcomes.
    • Coordinating and managing care for targeted patient populations across the care continuum to improve clinical outcomes, focusing on comprehensive, holistic care.
    • Collaborating effectively with physicians, medical assistants, and others to address SDOH concerns.
    • Assisting in the development and implementation of individualized care plans for patients, especially those with complex or chronic medical conditions.
    • TOC (Transitions of Care) coordination, including follow-up with patients discharged from hospitals or other facilities to ensure continuity of care and prevent readmissions.
    • Coordinating medical clearances for patients undergoing procedures or needing specialized care.
  • Quality Improvement & Reporting:
    • Playing a critical role in improving patient care by monitoring, reporting, and optimizing performance on quality metrics such as HEDIS, CMS Star Ratings, NCQA standards, and other payer-driven initiatives.
    • Analyzing and reporting on healthcare quality measures, identifying gaps in care and implementing evidence-based interventions to address them.
    • Collaborating with providers, interdisciplinary teams, and health plan partners to close gaps in care (e.g., ensuring appropriate screenings, optimizing chronic disease management).
    • Reviewing medical records and utilizing EMR data to track compliance with quality standards and identify areas for improvement.
    • Participating in audits, chart reviews, and data validation for accurate reporting to payers and regulatory bodies.
    • Leading or supporting quality improvement projects aligned with HEDIS, CAHPS, CMS, or ACO benchmarks to continuously enhance patient outcomes.
    • Maintaining detailed and accurate documentation in care management and quality tracking systems to ensure data integrity.
  • Clinical Operations & Education:
    • Management of the Lab, including overseeing specimen collection processes, ensuring proper handling, and coordinating with external lab services.
    • Management of the CLIA (Clinical Laboratory Improvement Amendments) laboratory program, ensuring the practice's laboratory operations comply with all federal and state regulations, including quality control, proficiency testing, and personnel requirements.
    • Management of the New York State Vaccines for Children (NYS VFC) Program, ensuring compliance with all program requirements for vaccine ordering, storage, handling, inventory management, documentation, and reporting to the New York State Immunization Information System (NYSIIS).
    • Family member education on health conditions, treatment options, preventive care, and medication management in a clear and empathetic manner.
    • Educating patients and providers on preventive health guidelines and promoting adherence to care plans.
  • Leadership: The Primary Care RN Coordinator will contribute to defining and executing strategies and solutions to create business value in nursing within Care Medical Practice PLLC.
  • Strategy: The Primary Care RN Coordinator will support the establishment of business strategy and roadmap to: (1) improve outcomes for Care Medical members; (2) enhance the efficacy of other Care Medical business divisions; and (3) develop and deliver external market opportunities for Care Medical products and services. In supporting the business strategy, the Primary Care RN Coordinator will contribute to defining and innovating sustainable revenue models to drive the profitability of the Company.
  • Collaboration: The Primary Care RN Coordinator will ensure that nursing capabilities form a cohesive offering, including by working closely with other business divisions to learn their needs, internalize their knowledge, and define solutions to achieve the business objectives of Care Medical Practice PLLC.
  • Knowledge: The Primary Care RN Coordinator will provide subject matter expertise in nursing solutions, including determining and recommended approaches for nursing, solution deployment, and performance evaluation.
  • Culture: The Primary Care RN Coordinator is accountable for creating a productive, collaborative, safe, and inclusive work environment for the nursing team and as part of the larger Company.

Required Qualifications

The ideal candidate for this Primary Care RN Coordinator position will possess the following qualifications:

  • Education: Bachelor’s or equivalent degree in Nursing with a record of strong academic achievement.
  • Licensure: Current and active Registered Nurse (RN) license in the State of New York.
  • Experience: At least 2 years of nursing experience, with a strong preference for candidates who have 2+ years of relevant healthcare experience in a primary care or outpatient setting, demonstrating familiarity with quality metrics such as HEDIS, CMS Star Ratings, or NCQA standards. Experience with lab management and care coordination is a plus. Specific experience with the NYS VFC Program and CLIA compliance is highly desirable.
  • Communication: Excellent verbal, written communication, and presentation skills; ability to clearly articulate and present complex concepts and models in an accessible manner to patients, families, colleagues, and other stakeholders.
  • Technical Proficiency: Proficiency in all Microsoft Office applications (Word, Excel, Outlook). Strong experience with Electronic Medical Records (EMR) systems is essential, MEDENT experience is a plus. Familiarity with quality tracking systems and the New York State Immunization Information System (NYSIIS) is highly desirable.

The working environment and physical requirements of the job include:

Practice-based, with daily In-office work. The job is performed indoors in a traditional office setting with air conditioning, artificial light, and an open workspace.

In this position you will need to communicate with customers, vendors, management, and other co-workers in person and over devices, sometimes with people who are agitated. Regular use of the telephone and e-mail for communication is essential. Sitting for extended periods is common. Must be able to receive ordinary information and to prepare or inspect documents. Lifting up to 10 lbs. occasionally may be required. Good manual dexterity for the use of common office equipment such as computer terminals, calculators, copiers, and FAX machines. Good reasoning ability is important. Able to understand and utilize management reports, memos, and other documents to conduct business.
