Overview
The Regional Director of Practice Transformation is a senior leadership role responsible for overseeing, managing, and developing a regional team of Practice Facilitators who support primary care practices participating in value-based care models. This role provides strategic direction, performance oversight, and people leadership to ensure consistent, high-quality execution of practice transformation initiatives across the assigned region.
The Regional Director collaborates closely with Network, Clinical, and Business leaders to develop and execute regional strategies that improve practice performance across value-based care contracts, including quality outcomes, utilization management, risk adjustment, and total cost of care. This position is accountable for regional results, team effectiveness, and alignment between frontline practice facilitation work and enterprise value-based care objectives.
Duties & Responsibilities
- People Leadership & Team Management
- Lead, manage, coach, and develop a regional team of Practice Facilitators.
- Establish clear expectations, goals, and performance standards aligned with organizational and contract-specific priorities.
- Conduct regular performance reviews, provide ongoing coaching, and support professional development and retention.
- Ensure appropriate staffing models, facilitator caseload distribution, and regional coverage.
Regional Strategy & Practice Transformation Oversight
- Develop and execute regional practice transformation strategies aligned with value-based care contracts and organizational goals.
- Translate enterprise-level value-based care objectives into regional priorities, work plans, and facilitator focus areas.
- Ensure consistent application of practice facilitation methodologies, tools, and best practices across all supported practices.
Collaboration with Network, Clinical & Business Leaders
- Collaborate closely with Network, Clinical, and Business leaders to identify performance gaps, risks, and opportunities across the region.
- Co-develop and prioritize interventions to improve quality performance, utilization, risk adjustment, and financial outcomes.
- Serve as a strategic advisor by synthesizing practice-level insights and data into actionable recommendations for senior leadership.
Performance Management & Executive Reporting
- Own regional performance outcomes related to quality measures, utilization, patient outcomes, and financial performance under value-based contracts.
- Review regional dashboards, facilitator reports, and practice performance data to guide decision-making and corrective actions.
- Prepare and deliver executive-level presentations, summaries, and performance readouts for senior leadership and business stakeholders.
Operational Excellence, Governance & Scalability
- Establish and maintain standard operating processes, tools, and reporting structures to support effective practice facilitation.
- Identify systemic barriers to practice performance and implement scalable, sustainable solutions.
- Ensure regional readiness for audits, payer reviews, and regulatory requirements in partnership with internal teams.
- Stay current on value-based care models, primary care transformation strategies, and industry best practices.
Qualifications
- Education:Bachelor’s degree required, BSN Preferred; Master’s degree in Public Health, Healthcare Administration, Nursing, or a related field preferred.
Experience:
- 7+ years of experience in healthcare transformation, value-based care, population health, or quality improvement.
- Demonstrated experience leading, managing, and developing teams.
- Experience working with or supporting primary care practices in value-based payment models.
Certifications (Preferred): Lean, Six Sigma, PMP, CPHQ, or other relevant quality, operations, or project management certifications.
Technical Skills:
- Experience with quality reporting systems, EHRs, and population health or analytics platforms.
- Proficiency in Microsoft Office and presentation tools.
Leadership & Professional Skills:
- Strong people leadership, coaching, and performance management skills.
- Executive presence with the ability to communicate complex information clearly to senior leaders.
- Strategic thinking combined with strong operational execution.
- Ability to manage multiple priorities in a fast-paced, evolving value-based care environment.
The working environment and physical requirements of the job include:
In-office work is performed indoors in a traditional office setting with air conditioning, artificial light, and an open workspace. Travel to practices is required up to 75% of the time
In this position you will need to communicate with customers, vendors, management, and other co-workers in person and over devices, sometimes with people who are agitated. Regular use of the telephone and e-mail for communication is essential. Sitting for extended periods is common. Must be able to receive ordinary information and to prepare or inspect documents. Lifting of up to 10 lbs. occasionally may be required. Good manual dexterity for the use of common office equipment such as computer terminals, calculator, copiers, and FAX machines. Good reasoning ability is important. Able to understand and utilize management reports, memos, and other documents to conduct business.
