Position Overview

The Per Diem Field Registered Nurse reports to the Clinical Manager or Director of Clinical Operations. This flexible role allows you to provide compassionate, evidence-based care on an as-needed basis. As part of our Care at Home model, you’ll support high-risk patients, conducting assessments, coordinating care, and ensuring that patient outcomes are prioritized in collaboration with caregivers.

Responsibilities

  • Perform in-home, virtual, or telehealth assessments based on member needs and clinical leadership direction.
  • Develop and implement individualized nursing care plans in collaboration with patients and caregivers.
  • Respond to clinical escalations in real-time via phone, virtual visits, or in-person home visits.
  • Track and update care plan outcomes and reassess patient needs as necessary.
  • Provide chronic disease education, care coordination, and support for patients with complex medical and mental health conditions.
  • Facilitate gap closures in care following evidence-based guidelines.
  • Document all activities in the electronic medical record system in compliance with policies and procedures.
  • Collaborate with interdisciplinary teams to ensure quality outcomes for patients and caregivers.
  • Engage patients in managing their health proactively, including medication adherence and follow-up appointments.
  • Provide subject matter expertise and support for best clinical practices.
  • Maintain a positive, collaborative, and inclusive working environment.

Qualifications

  • Education: BSN required; MSN or other advanced healthcare degree preferred.
  • Active, unrestricted RN license in the state of hire (eNLC license preferred).
  • Experience: 3-5 years of relevant clinical experience; care management experience in a health plan, hospital, home health, or hospice setting is a plus.
  • BLS certification.
  • Valid driver’s license, auto insurance, and reliable transportation.
  • Experience with EMRs and Microsoft Office Suite.
  • Strong assessment, analytical, and critical-thinking skills.
  • Effective problem-solving and decision-making abilities.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and adapt to a flexible schedule.

Preferred Qualifications

  • Knowledge of HCC documentation, ICD-10 Coding, Health Risk Assessments, HEDIS, and STAR quality measures.
  • Experience in geriatric care and advanced care planning.
  • Bilingual proficiency is a plus.

Schedule

This is a per diem position offering flexible scheduling based on organizational need and nurse availability. Shifts may include weekdays, weekends, and holidays, with the potential for on-call responsibilities.

Location

Field with travel to Family members’ homes and clinical settings.

The working environment and physical requirements of the job include:

This position requires in-home, assisted living, and independent-living community based work. The job requires frequent travel for patient visits in all types of weather conditions. Work may be performed in settings with conditioned air, artificial light, and an open workspace.

In this position you will need an ability to travel frequently by car and/or public transportation, the ability to communicate with customers, vendors, management, and other co-workers in person and over devices, sometimes with people who are agitated. Regular use of the telephone and e-mail for communication is essential. Sitting or standing for extended periods is common. Must be able to receive ordinary information and to prepare or inspect documents. Lifting of up to 50 lbs. occasionally may be required. Good manual dexterity for the use of common office equipment such as computer terminals, calculator, copiers, and FAX machines. Good reasoning ability is important. Able to understand and utilize management reports, memos, and other documents to conduct business.
