Overview
The Vice President of National Initiatives will report to the Senior Vice President of Population Health. This role will be accountable for providing evidenced-based analysis, strategy and optimized processes to facilitate performance in population health initiatives including utilization management, medication adherence, and social determinants of health fulfillment. This individual will lead the overall team of Social Workers, Clinical Pharmacists, Transition of Care nurses, and initiative leaders.
Duties & Responsibilities
- The National Initiatives leader will develop the strategy and manage the teams the provide national population health team inclusive but not limited to social workers, transition of care, and clinical pharmacists.
- Drive strategy around improvement opportunities, in partnership with national and regional leadership to improve outcomes in quality and clinical outcomes.
- Drive reduction in medical expense through reduction in readmission rates through TOC and UM teams, reduction in polypharmacy, and SDOH team fulfilment.
- Set performance expectations with team, based on contractual obligations, roles and responsibilities.
- Fosters a team that can build trust and buy- in with key stakeholders and care teams to facilitate change management, engagement, solutioning, and sustainability of population health initiatives.
- Support Quality and Risk Adjustment improvement initiatives by assessing current state and collaborates with key stakeholders on population health efforts.
- Develop and implement strategies and KPI for medication adherence and pharmacy teams to hit 4 STARS on medication adherence measures and other pharmacy HEDIS/STAR measures.
- Displays strong time management and communication skills that align with CINQCARE’s core values.
- Monitor, interpret, and track HEDIS and other quality performance measures with a focus on SDOH fulfilment and transition of care measures, at a team member, practice, market and national level, to optimize care outcomes and maximize revenue.
- Drives success in population health performance by guiding teams to achieve best practice standards/benchmarks by providing ongoing engagement, training, and coaching.
- Possesses a solid knowledge of internal enterprise-wide contractual areas of focus, Medicaid VBP and Medicare entities to monitor and report metrics such as utilization trends, patient satisfaction, and other key performance indicators
- Leverages internal and external resources to meet family member SDOH needs at the national and market level.
- Ensure team members can:
- Provide telephonic transition of care management that reduces unnecessary readmissions
- Identify and solution for SDOH needs of family members
- Provide clinical pharmacy support including medication reconciliation, polypharmacy review, consultation and support medication adherence initiatives.
- Collaborate with market and national care delivery teams to ensure care coordination
- Provide family members service that aligns with the values of CINQCARE
Qualifications
- Education: BSN, MSN or equivalent degree
- Certifications: BSN, NP, or PA
- Experience:
- 5+ years of clinical leadership experience
- 5+ years of Medicare/Medicaid experience
- 2+ years of telephonic transition of care team experience
- Experience in Quality Improvement, and/or facilitation skills preferred.
- Experience with analytical skills and data analysis plus experience of HEDIS/STARS
- Leading telephonic transition of care team to reduce readmission rates.
- Experience with medication adherence and clinical pharmacy programs
- Technical Skills:
- Excellent computer skills required particularly related to Microsoft applications including Word. Excel, PowerPoint, and Outlook.
- Experience with PowerBI preferred
- Willingness required to explore and learn new or unfamiliar applications and databases, and when prepared to teach team members how to maximize technology
- Soft Skills:
- Continuously add value as an innovative thinker, active listener, with the ability to build and cultivate relationships.
- Possess and strong business acumen with the ability to anticipate the needs of market and leadership partners
- Possess excellent organizational, problem-solving, and analytical skills, while coordinating multiple projects.

The working environment and physical requirements of the job include:
Work is performed indoors in a setting with air conditioning and artificial light. Travel to and work in offices or other environments is required.
In this position you will need to communicate with customers, vendors, management, and other co-workers in person and over devices, sometimes with people who are agitated. Regular use of the telephone and e-mail for communication is essential. Sitting for extended periods is common. Must be able to receive ordinary information and to prepare or inspect documents. Lifting of up to 10 lbs. occasionally may be required. Good manual dexterity for the use of common office equipment such as computer terminals, calculator, copiers, and FAX machines. Good reasoning ability is important. Able to understand and utilize management reports, memos, and other documents to conduct business.
