About You

The Program Manager will have the following qualifications:

  • Education. Nursing degree, either RN or NP, certification in Obstetrical care preferred, or bachelor’s or master’s degree in public health.
  • Experience. Minimum of 5 years of experience in a health care delivery setting or managed care setting with experience in program development and a track record in program implementation.
  • Skills. Must be

o Able to remain on task and work independently.

o Organized, reliable, and attentive to detail.

o Proficient in Excel, Word, and PowerPoint.

About the Job

The Program Manager for CINQCARE for Moms reports to the Director of CINQCARE for Moms and serves as an integral part of the Care at Home & Cinqcare team participating in the operational launch of new maternal health programs. The Program Manager is accountable for driving flawless execution of the maternal health program by coordinating with internal resources, external stakeholders, and third parties/vendors. The Program Manager must ensure that all projects are delivered timely and within scope and serve as an integral part of the CINQCARE Connect team. The Program Manager for CINQCARE for Moms should embody CINQCARE’s core values, including, Trusted, Empathetic, Committed, Humble, Creative and Community-Minded.

Primary Responsibilities

The Program Manager for CINQCARE for Moms will have the following responsibilities:

  • Manages activities to achieve CINQCARE’s strategic goals and initiatives on the Maternal and Infant Health Program.
  • Develop work plans and execute the activities to implement the maternal care program.
  • Utilizes financial and clinical acumen, develops efficient clinical objectives, standards of care, policies and procedures, patient improvement programs and educational in-services, and workflows, within and outside of the Programs to foster high-quality care.
  • Identifies internal business owners and external partnerships required to support existing and new business requirements.
  • Escalates to the leadership team barriers to completion of tasks and lack of timely sub-project completion.
  • Establishes and manages dashboards that measure operational effectiveness and efficiency, clinical quality, and member experience of assigned practices. Develop and execute strategic action plans aimed at improving metrics that are not meeting targets. Provides dashboard and action plan results monthly.
  • Utilizes data-driven insights to make informed decisions and adjustments to strategies.
  • Identification of potential risks and development of contingency plans to mitigate them.
  • Monitors progress of Key Performance Indicators (KPIs) and tracks performance goals
  • Prepares reports and presentations to communicate progress and results.
  • Acts as a liaison between departments.
  • Ensures the coordination of care across Care at Home services for women including reproductive health, family planning, and perinatal care by connecting women to important services and resources (e.g., OB/GYN, primary care, oncology, cardiology, breast imaging, behavioral health).
  • Supports ongoing collaboration with tertiary and other providers ensuring that resources are utilized appropriately to the full extent.
  • Assesses, plans, implements, and evaluates community outreach directed at women’s health needs through education programs.
  • Participates in obstetric, newborn, and other clinical quality meetings at the regional, state, and national level.
  • Responsible for hiring, orienting, training, ongoing education, evaluating, and supervising clinical and non-clinical staff.
  • Participates in preparation of department budgets. Ensure that fiscal responsibility is exercised in all financial decisions.
  • Performs other duties as assigned or requested.

This position requires both in-home and office-based work. In-office work is performed indoors in a traditional office setting with conditioned air, artificial light, and an open workspace.

In this position you will need the ability to communicate with customers, vendors, management, and other co-workers in person and over devices, sometimes with people who are agitated. Regular use of the telephone and e-mail for communication is essential. Sitting for extended periods is common. Must be able to receive ordinary information and to prepare or inspect documents. Lifting of up to 30 lbs. occasionally may be required. Good manual dexterity for the use of common office equipment such as computer terminals, calculator, copiers, and FAX machines. Good reasoning ability is important. Able to understand and utilize management reports, memos, and other documents to conduct business.
