About You

The Practice Transformation Facilitator (PTF) should have the following qualifications:

  • Education. Graduate from an accredited program (diploma, certificate, associate degree, bachelor’s degree) with a record of strong academic achievement.
  • Experience. At least 3 years of clinical practice experience. Ideal candidates will have 5+ years of relevant clinical experience in the healthcare industry; Previous experience in healthcare settings, particularly in primary care practices with successful implementation of quality improvement initiatives, is advantageous.
  • Entrepreneurial. CINQ seeks to fix gaps that have persisted for generations in the delivery of care to Black and Brown populations. This position is accountable for ensuring CINQ is positioned to innovatively deliver on its promise; Demonstrated ability to execute tasks with a sense of urgency and drive results in a dynamic healthcare environment.
  • Communication. Excellent verbal, written communication and presentation skills; ability to clearly articulate and present concepts and models in an accessible manner to CINQ’s team, investors, partners and other stakeholders. Proficiency in all Microsoft Office applications; Ability to interact professionally with diverse stakeholders, including healthcare facilities, staff, and community partners.
  • Relationships. Ability to build and effectively manage relationships with business leaders and external constituents; and,

About the Job:

The Practice Transformation Facilitator reports to the ACO Market Physician Leader, with accountability for providing strategy, judgment, organization, and evidenced-based analysis to influence decisions, and meeting CINQCARE requirements. They should embody CINQCARE’s core values, including, Trusted, Empathetic, Committed, Humble, Creative and Community-Minded.

The PTF will have the following responsibilities:

  • Support Provider Engagement by supporting providers and practice staff in ACO Programs and Initiatives
  • Support Patient Engagement through outreach and scheduling within Provider Practices
  • Work to educate and support office support staff in patient outreach, engagement, ACO education, scheduling, and referral to care programs
  • Coordinate with a team of Practice Transformation Facilitators/Practice Transformation Providers across various practice settings.
  • Assist and support practices in optimizing available information technology resources to enhance patient outcomes and office efficiencies.
  • Collaborate with interdisciplinary teams to identify opportunities to support provider practices, in:
  • High-Risk patient outreach, scheduling and engagement/referral to care programs
  • Care coordination workflows to improve transitions of care coordination and scheduling, AWV scheduling, and high-risk patient scheduling
  • Implementation of care management strategies to enhance patient care quality and outcomes
  • Foster a collaborative and supportive environment for providers and patients in the ACO
  • Serve as the primary liaison between providers, clinics, facilities such as SNF or Hospital, and the ACO Team
  • Provide guidance and support to care teams, including physicians and staff, in adopting best practices, sharing performance gaps, and implementing quality improvement strategies.
  • Build and maintain trust-based relationships with healthcare practices and care teams to facilitate change management, engagement, and sustainability of transformation initiatives.
  • Deliver progress reports to healthcare practices and care teams, identifying performance gaps and providing guidance on solutions.
  • Prioritize key quality and utilization initiatives critical to success under value-based care programs.
  • Provide training and guidance on the use of technology tools to healthcare practice teams as needed
  • Document and assess practice progression throughout the transformation process.

The working environment and physical requirements of the job for the Practice Transformation Facilitator include:

This position requires on-site work. The job is performed indoors in a traditional office setting with conditioned air, artificial light, and an open work space.

In this position you will need an ability to communicate with office support staff, providers, facility staff, patients, and interdisciplinary teams in person and over devices. Regular use of the telephone and e-mail for communication is essential. Sitting for extended periods is common. Must be able to receive ordinary information and to prepare or inspect documents. Lifting of up to 10 lbs. occasionally may be required. Good manual dexterity for the use of common office equipment such as computer.
