Patient Information Coordinator

The Patient Information Coordinator compiles, processes and maintains patient medical records consistent with medical, administrative, ethical, legal, and regulatory requirements of the health care system. This individual will maintain accurate filing of patient records to ensure quality medical care of our patients, while maintaining strict confidentiality of patient data. Discretion, high integrity, organization, and attention to detail are all critical success factors for this role.

The Patient Information Coordinator will have the following responsibilities:

  • Reviewing incoming loose documents and scanning into appropriate patient’s EMR (or faxing to Athena) to be reviewed and signed off by physician.
  • Completing requests for copies of Medical Records and mailing, accordingly, also completing charge record/invoice for Medical Record copies as appropriate.
  • Checking incoming faxes and distributing documents received, as necessary.
  • Protecting confidentiality of all patient records
  • Monitoring practice’s clinical inbox in the EMR system, reviewing and assigning documents as appropriate.
  • Processing all incoming mail
  • Ability to perform chart abstraction
  • Coordinating with third party vendor to ensure Home Health orders are processed and signed by physician.
  • Other duties as assigned

About You

The Patient Information Coordinator should have the following qualifications:

Education and Experience:

  • High school diploma or equivalent required.
  • 2 – 5 years’ medical office and/or records filing experience preferred.
  • Demonstrated ability to maintain confidentiality.
  • Great attention to detail and strong organizational skill.

The working environment and physical requirements of the job include:

This position requires both in-home and office-based work. In-office work is performed indoors in a traditional office setting with conditioned air, artificial light, and an open workspace.

In this position you will need an ability to communicate with customers, vendors, management, and other co-workers in person and over devices, sometimes with people who are agitated. Regular use of the telephone and e-mail for communication is essential. Sitting for extended periods is common. Must be able to receive ordinary information and to prepare or inspect documents. Lifting of up to 30 lbs. occasionally may be required. Good manual dexterity for the use of common office equipment such as computer terminals, calculator, copiers, and FAX machines. Good reasoning ability is important. Able to understand and utilize management reports, memos, and other documents to conduct business.
