Overview
This position reports to the Office Manager. The Intake Coordinator facilitates the onboarding for homebound or home-limited patients and coordinates the initial visit with the patient/family/referral source to begin to provide quality healthcare. To accomplish this, we're looking for individuals who share our sense of excellence and never settle for the status quo. Duties & Responsibilities
- Patient Intake: Collect and verify essential information from patients, ensuring all forms and documentation are completed accurately.
- Schedule Coordination: Manage scheduling of assessments and services and communicate appointment details to patients.
- Eligibility Verification: Verify insurance coverage and eligibility, ensuring proper documentation for billing and services.
- Collaboration with Teams: Work with medical and administrative staff to ensure smooth coordination of care and accurate information transfer.
- Record Maintenance: Maintain accurate patient records in EHR systems and ensure proper documentation storage.
- Customer Service & Problem-Solving: Address patient concerns, provide excellent service, and resolve any issues related to the intake process.
Qualifications
- Education: High School Diploma/Equivalency required.
- Experience:
- Experience in primary care, family practice, internal medicine, urgent care or ER preferred.
- Adept in medical terminology and insurance practices.
- Proficient in use of computer programs and applications including Outlook and Excel applications and practice EMR systems.
- Communication: Excellent verbal, written communication and presentation skills; ability to clearly articulate and present concepts and models in an accessible manner to Grace at Home’s team, investors, partners, and other stakeholders. Proficiency in all Microsoft Office applications.
- Relationships: Ability to build and effectively manage relationships with business leaders and external constituents.
- Culture: Good judgement, impeccable ethics, and a strong team player; desire to succeed and grow in a fast-paced, demanding, and entrepreneurial Company.
The working environment and physical requirements of the job include:
In-office work is performed indoors in a traditional office setting with air conditioning, artificial light, and an open workspace.
In this position you will need to communicate with customers, vendors, management, and other co-workers in person and over devices, sometimes with people who are agitated. Regular use of the telephone and e-mail for communication is essential. Sitting for extended periods is common. Must be able to receive ordinary information and to prepare or inspect documents. Lifting of up to 10 lbs. occasionally may be required. Good manual dexterity for the use of common office equipment such as computer terminals, calculator, copiers, and FAX machines. Good reasoning ability is important. Able to understand and utilize management reports, memos, and other documents to conduct business.
