Overview
The Manager of Value-Based Care Education & Practice Collaboratives leads the design, development, and delivery of comprehensive education strategies for both primary care practices, Practice Facilitators, and internal quality/risk teams supporting value-based care arrangements across the enterprise. This role ensures internal teams, providers, practice teams, and facilitators are equipped with the knowledge, tools, standardized processes, and best practices needed to succeed across quality performance, risk adjustment, utilization management, total cost of care, and population health domains.
This leader translates complex regulatory, payer, and operational requirements into practical, actionable education and training that drives measurable performance improvement. The role collaborates closely with Clinical, Network, Quality, Risk, Practice Transformation, Analytics, and Business teams to ensure alignment with organizational priorities, payer contracts (including ACO and MSSP requirements), and enterprise performance goals.
Duties & Responsibilities
Education & Learning Collaborative Strategy
- Design and lead a comprehensive education and training strategy supporting both primary care practices, Practice Facilitators, and internal teams on value-based care models.
- Align curricula and collaboratives with payer contracts, ACO and MSSP requirements, HEDIS/STAR measures, risk adjustment, utilization management, and total cost of care strategies.
- Translate regulatory, clinical, operational, and contractual requirements into practical guidance and standardized training.
- Ensure alignment of education initiatives with organizational strategy and identified performance improvement priorities.
Curriculum Development & Standardization
- Develop and maintain standardized curricula, playbooks, toolkits, job aids, policies, and SOPs to support both practice education and facilitator training.
- Design orientation, onboarding, ongoing education, and advanced skill development programs for Practice Facilitators.
- Partner with subject matter experts across Clinical, Quality, Risk, Analytics, and Operations to ensure content reflects current best practices and enterprise priorities.
- Maintain centralized repositories of education and training materials, ensuring version control, accessibility, and compliance with documentation standards.
- Continuously update content to reflect evolving value-based care requirements and organizational initiatives.
Learning Collaboratives & Training Delivery
- Develop and oversee learning collaboratives, peer forums, and structured educational series to support continuous learning and practice transformation.
- Deliver live and virtual training sessions, workshops, and applied learning experiences for providers, practice teams, Practice Facilitators, and internal teams.
- Facilitate role-based, scenario-driven learning to build confidence and effectiveness in real-world practice settings.
- Provide reinforcement training, coaching, and follow-up sessions to support sustained application of learned concepts.
- Support onboarding and ramp-up of new Practice Facilitators to ensure readiness for field deployment.
Performance Enablement & Continuous Improvement
- Align education and training initiatives with identified gaps in quality, risk adjustment, utilization, and total cost of care performance.
- Use practice-level, facilitator-level, and population-level data to inform priorities and tailor learning experiences.
- Assist senior leadership in developing and tracking Enterprise level KPIs related to training reach, engagement, facilitator readiness, and impact on practice performance.
- Evaluate program effectiveness through feedback, assessments, and measurable performance outcomes.
- Identify best practices and emerging trends in practice facilitation and value-based care and incorporate them into training programs.
- Support change management efforts related to new programs, tools, and contractual requirements.
Collaboration & Stakeholder Engagement
- Collaborate closely with Practice Transformation leadership, Network, Clinical, Quality, Risk, Analytics, and Business teams to ensure education and training efforts to support enterprise strategies.
- Partner with regional and local leaders to coordinate delivery and maximize engagement.
- Serve as a trusted resource to providers, practice leaders, and Practice Facilitators on value-based care education, transformation strategies, and performance improvement.
Program Management & Execution
- Independently manage the design, development, and deployment of education programs and internal training initiatives alongside senior leadership vision
- Establish processes, timelines, governance standards, and quality controls for curriculum development and delivery.
- Monitor participation, engagement, and outcomes across learning collaboratives and facilitator training programs.
- Drive consistency across regions while allowing flexibility to address local payer and market requirements.
Qualifications
Education: Bachelor’s degree required; Master’s degree in Healthcare Administration, Healthcare Education, Public Health, Nursing, or related field preferred. An equivalent combination of education and relevant experience may be considered.
Experience:
- 5+ years of experience in practice facilitation, healthcare transformation, quality improvement, value-based care, or related field.
- Experience working with primary care practices and interdisciplinary care teams.
- Demonstrated subject matter expertise in value-based payment models including quality programs, risk adjustment, utilization management, and population health.
- Experience developing and delivering education, workflows, and training programs for adult learners.
- Experience facilitating group learning sessions, collaboratives, or professional education programs.
- Familiarity with common EMR systems such as Athena, NextGen, Epic or experience with EMR analytics/template development a plus
Skills:
- Strong presentation, facilitation, and instructional design skills.
- Ability to translate complex healthcare concepts into clear, actionable education.
- Excellent communication and relationship-building skills with providers and practice leaders.
- Proficiency in Microsoft Office and virtual learning platforms.
- Ability to manage multiple initiatives and timelines in a fast-paced environment.
The working environment and physical requirements of the job include:
Work is performed indoors in a setting with air conditioning and artificial light. Travel to and work in offices or other environments is required.
In this position you will need to communicate with customers, vendors, management, and other co-workers in person and over devices, sometimes with people who are agitated. Regular use of the telephone and e-mail for communication is essential. Sitting for extended periods is common. Must be able to receive ordinary information and to prepare or inspect documents. Lifting of up to 10 lbs. occasionally may be required. Good manual dexterity for the use of common office equipment such as computer terminals, calculator, copiers, and FAX machines. Good reasoning ability is important. Able to understand and utilize management reports, memos, and other documents to conduct business.
