Position Overview

The Family Nurse Practitioner (FNP) at Care Medical Practice PLLC plays a pivotal role in delivering comprehensive primary care to patients across the lifespan—from pediatrics to geriatrics. Reporting to the Medical Director, the FNP provides clinical leadership and direct care services that reflect a deep commitment to health equity and quality outcomes. This role blends independent clinical judgment with care coordination expertise, with an emphasis on preventative care, chronic disease management, and patient empowerment in both in-office and community-based settings.

Key Responsibilities

As a Family Nurse Practitioner, you will be responsible for:

Patient Assessment, Diagnosis & Treatment

  • Conducting comprehensive physical examinations of pediatric and adult patients, including reviewing medical histories and performing focused assessments.
  • Ordering and interpreting diagnostic tests (e.g., laboratory panels, X-rays, ultrasounds) to aid in clinical decision-making.
  • Diagnosing and managing a broad range of acute and chronic conditions, incorporating evidence-based best practices.
  • Prescribing medications and treatments in accordance with New York State regulations and care protocols.
  • Providing routine and preventive care services, including immunizations, screenings, and health counseling.
  • Delivering patient and family education related to disease management, epidemic awareness, and health promotion.
  • Collaborating with physicians, behavioral health, nursing staff, and community stakeholders to coordinate care for complex cases.
  • Overseeing follow-up care and long-term health planning to promote sustained health improvement and minimize disparities.

Quality Outcomes & Professional Development

  • Supporting performance on quality measures such as HEDIS, CMS Star Ratings, and NCQA standards through proactive gap closure and documentation.
  • Maintaining accurate and timely documentation in the Electronic Medical Record (EMR) and other reporting systems.
  • Participating in audits, research initiatives, and continuous learning to advance clinical practice and improve outcomes.
  • Promoting patient-centered care and cultural humility in every encounter, aligning with Care Medical’s mission to remove barriers to health.

General Duties

Leadership: The FNP will contribute clinical and strategic leadership within the practice to ensure an integrated approach to patient care, reflecting innovation, compassion, and accountability.

Strategy: The FNP will help guide practice growth and clinical model design by identifying emerging health trends and proposing strategies that align with Care Medical Practice’s goals for high-quality, sustainable care delivery.

Collaboration: The FNP will work cross-functionally with operational, administrative, and community-based teams to ensure a coordinated and effective care ecosystem.

Knowledge: The FNP brings advanced clinical expertise in primary care for pediatric and adult populations and contributes to practice protocols, peer development, and patient engagement strategies.

Required Qualifications

  • Education:
    • Current, unrestricted NP or PA license to practice in New York State.
    • Graduate of an accredited Physician Assistant program or Nurse Practitioner program required.
    • Board Certification from ANCC or AANP (Nurse Practitioner) or NCCPA (Physician’s Assistant).
    • Prescriptive authority/authorization in the State of New York including the ability to obtain DEA controlled substance registration.
    • Valid NPI and American Heart Association BLS Certification.

The working environment and physical requirements of the job include:

This position requires on-site work. The job is performed indoors in a traditional office setting with conditioned air, artificial light, and an open workspace.

In this position you will need an ability to work onsite, communicate with customers, vendors, management, and other co-workers in person and over devices, sometimes with people who are agitated. Regular use of the telephone and e-mail for communication is essential. Sitting for extended periods is common. Must be able to receive ordinary information and to prepare or inspect documents. Lifting up to 10 lbs. occasionally may be required. Good manual dexterity for the use of common office equipment such as computer terminals, calculators, copiers, and FAX machines. Good reasoning ability is important. Able to understand and utilize management reports, memos, and other documents to conduct business.
