About You

Education & Experience to drive change:

  • Medical Assistant Certification required
  • 3+ years of experience in a healthcare setting or a bachelor’s degree in a healthcare related field.
  • High School Diploma or GED Required
  • Comfort with technology including the Microsoft suite of products
  • Prior experience using electronic health record.

About the Job

We are looking for a Care Coordinator- Medical Assistant to help with the way primary care is delivered and how patients are served. The Care Coordinator facilitates the delivery of care by the Provider. Care Coordinators are liaisons to the patient/family/caregiver working with the Provider's direction and orders to provide quality healthcare for the home bound or home limited patients.

While this role will be dynamic in nature, key responsibilities and expectations of the position include:

  • Order any acute imaging/labs for Provider
  • Answers phone calls/messages from or pertaining to Grace At Home patients
  • Provide support to designated provider in all patient care areas
  • Communicating & collaborating with other Care Coordinators to assure all work is completed
  • Keep workspace within HIPPA compliance
  • Communicate daily with Provider to encourage delivery of comprehensive care plan to each patient within the team and expedite workflow to facilitate Providers ability to see patients
  • Check EHR messages regularly. Follow up on requests in a timely manner
  • Send in prescription refills
  • Communicate with community partners to facilitate care for patients
  • Processing inactive patient information

The working environment and physical requirements of the job include:

This position requires both in-home and office-based work. In-office work is performed indoors in a traditional office setting with conditioned air, artificial light, and an open workspace.

In this position you will need an ability to communicate with customers, vendors, management, and other co-workers in person and over devices, sometimes with people who are agitated. Regular use of the telephone and e-mail for communication is essential. Sitting for extended periods is common. Must be able to receive ordinary information and to prepare or inspect documents. Lifting of up to 30 lbs. occasionally may be required. Good manual dexterity for the use of common office equipment such as computer terminals, calculator, copiers, and FAX machines. Good reasoning ability is important. Able to understand and utilize management reports, memos, and other documents to conduct business.
