Responsibilities:

The Program Coordinator ensures effectiveness of all participant-related program activities in conjunction with program management. In addition, the incumbent assures effective utilization of all community health workers.

The responsibilities are summarized as:

  • Address intake and assessments, services planning, service delivery and goal attainment
  • Provide training, supervision and problem –solving for difficult cases
  • Adhere to grant objectives by providing information to program managers highlighting progress to reaching goals
  • Manage day-to-day operations of community health workers participant  goals, objective and treatment plans; maintains a limited caseload
  • Conducts weekly case conferencing and monthly case files audits to assure participant records are complete and accurate
  • Monitor program compliance to VOALA policies and required procedures; maintains documentation of all program procedures
  • Coordinate outreach and collaboration efforts with community partners and community health centers
  • Coordinate supportive services for Medi-Cal eligible adults with chronic or complex health and/or behavioral health conditions
  • Assist program manager with implementing policies, procedures and forms that facilitate the effective operation of intensive case management and delivery of high quality Whole Person Care services
  • Other duties as assigned

Requirements:

  • Bachelor's degree preferred
  • At least 3 years of experience working with re-entry population, individuals experiencing homelessness, and/or special needs populations
  • Prior experience as a supervisor or team leader managing (5) or more individuals with diverse backgrounds
  • Demonstrated proficiency in MS Word and Excel as well as Internet use
  • Must be able to pass a background check
  • Valid California driver’s license, insurance and reliable transportation
  • Effective written and oral communications skills

  • Must be sensitive to the needs of those who are recovering or dually-diagnosed and be able to work on a recovery team
  • Ability to work with people from various backgrounds and/or limited English capabilities
  • Experience in event planning/community engagement
  • Knowledge about community resources
  • Experience connecting clients to resources
  • Must be willing to travel 25% of the time
