Community Health Workers should have a history with incarceration and have demonstrated a period of stability and successful reintegration into the community following incarceration.

Responsibilities:

  • Provide in reach/outreach and engagement to the homeless community through reports/referrals from Los Angeles County Jails, Los Angeles Sheriffs Department, social services providers and others
  • Conduct comprehensive screenings and assessments with homeless individuals to develop a case plan
  • Mentors, motivates and empowers client to successfully re-enter community by establishing a trusting and open relationship with client
  • Provides health education and counseling around disease management, medication adherence, mental health and SUD care, and self-care
  • Identifies community resources appropriate to meet clients' needs as identified by re-entry care plan
  • Assists client in navigating health system through appointment accompaniment, transportation assistance and coaching
  • Works with community-based re-entry programs and services to link clients to housing, employment, and other social services
  • Provide supportive counseling and advocacy for individuals
  • Other duties as assigned

Requirements:

  • High school diploma (or GED/High School Equivalence Certificate)
  • Proficiency with Microsoft Office; computer database experience
  • Valid California driver's license, insurance and reliable transportation
  • Effective written and oral communication skills

Additional Preferred:

  • Experience working with re-entry population, individuals experiencing homelessness, and/or special needs populations preferred
  • Personal experience with incarceration; knowledge of the barriers to re-entry to society preferred
  • Personal experience in recovery from mental health and/or substance abuse issues preferred
  • Knowledge of domestic violence, poverty, and federally funded mainstream programs preferred
  • Developing Relationships with families as well as variety of services stakeholders (schools, health and public benefit agencies)
  • Bilingual (Spanish) preferred
