Outreach Worker -Jail in Reach (R-ICMS) - West Covina

We provide a variety of social services to Los Angeles area communities such as Head Start programs, Upward Bound college prep programs, veterans’ services, homeless shelters, low-income housing program as well as drug and alcohol rehabilitation. Outreach Worker should have a history with incarceration and have demonstrated a period of stability and successful reintegration into the community following incarceration.

Responsibilities:

  • Assist with health system navigation, including:
  • Help making health appointments(primary care, specialty care, behavioral health, SUD treatment)
  • Accompaniment to health appointments
  • Assisting with understanding health care
  • Assist with social services navigation, including benefits and housing, as well as social supports, employment, legal, and compliance with Probation/Parole supervision, if applicable
  • Conduct comprehensive screenings and assessments  to develop a case plan
  • Provide crisis intervention if needed, and make referrals to appropriate community resources
  • Maintain complete client records, daily activity logs, mileage logs, CHAMP and other reports as directed
  • Other duties as assigned

Requirements:

  • Lived experience
  • 1-3 years’ experience working with homeless families/individuals or people in recovery
  • Developing relationships with community providers as well as a variety of service stakeholders (school, health and public benefit agencies)

Additional Preferred:

  • Proficiency with Microsoft Office; HMIS database experience strongly preferred
  • Valid California driver’s license, insurance and reliable transportation preferred
  • Effective written and oral communication skills; bilingual (Spanish) preferred
