Outreach Specialist - (Jail In Reach)

This jail in-reach program connects chronically homeless, dual diagnosed, frequently incarcerated individuals with a permanent housing solution. Embraces a “Housing First” approach, which prioritizes quick access to permanent housing without requiring “housing readiness”. Multi agency and inter-disciplinary provides wrap-around support. Connecting the most vulnerable to housing. Housing placement, employment services, mental health services, benefit services, case management, drug and alcohol treatment.

Responsibilities:

  • Provide in reach/outreach and engagement to the homeless community through reports/referrals from Los Angeles County Jails, Los Angeles Sheriff’s Department, social services providers and others
  • Conduct comprehensive screenings and assessments with homeless individuals to develop a case plan
  • Provide crisis intervention if needed, and make referrals to appropriate community resources
  • Provide supportive counseling and advocacy for individuals
  • Maintain complete client records, daily activity logs, mileage logs, HMIS and other reports as directed
  • Other duties as assigned

Requirements:

  • Bachelor’s Degree in social services or related field or equivalent combination of education and work experience
  • 1 - 3 years experience working homeless families/individuals or people in recovery
  • Knowledge of domestic violence, poverty, and federally funded mainstream programs
  • Developing Relationships with families as well as variety of services stakeholders (schools, health and public benefit agencies)
  • Excellent oral, written, organizational, and motivational skills
  • Must clear a LASD background clearance

Additional Preferred:

  • Proficiency with Microsoft Office; HMIS database experience strongly preferred
  • Valid California driver’s license, insurance and reliable transportation preferred
  • Effective written and oral communication skills; bilingual (Spanish) preferred
  • Veteran preferred
