VS–90 provides transitional housing to male Veterans who are on parole or probation. The goal of the program is for the client to transition into independent housing with sound financial planning skills and reduced institutional thinking. Services include case management, individual and group therapy, job referrals, and counseling regarding benefits and money management. This is a sober living program lasting six months (can be extended to 2 years).

Responsibilities:

This position requires certification in good standing as a Certified Nursing Assistant (CNA) with a preference for a Medication Technician Certification

The Certified Monitor:

  • Monitor medications and treatments as directed by VOA supervisor
  • Assure safe storage and record keeping of all medications in a transitional housing program for dually diagnosed, formerly homeless Veterans
  • Accurately document in the resident's chart, specifically related to medication management
  • Inform the supervisor and VA treatment team of any change in the resident's condition in a timely manner, and immediately report any suspicion/allegation of resident neglect/abuse
  • Coordinate with Veterans obtaining new medication and refills with VA Pharmacy services and/or program staff for timely administration of medication
  • Maintain confidentiality of department, resident and employee information, communication and documentation, as well as compliance with HIPAA regulations
  • Facilitate weekly health and wellness educational groups for program participants

Requirements:

  • High school diploma (or GED/High School Equivalence Certificate)
  • Certified Nursing Assistant (CNA) required: Medication Technician Certificate Preferred
  • Minimum 2 years' experience as Certified Nursing Assistant
  • Able to communicate clearly, effectively, and compassionately with others

Additional Requirements:

  • Knowledge of psychotropic medications and medications with abuse potential
  • If recovering alcohol and/or drug user, must have at least 1 year of continuous sobriety
  • Knowledge of chronically mentally ill, alcohol and drug abusing homeless and ways to motivate and engage them in medication and harm reduction
  • Successful completion of background screening
