Lead Case Manager -Whole Person Care - Compton

We provide a variety of social services to Los Angeles area communities such as Head Start programs, Upward Bound college prep programs, veterans’ services, homeless shelters, low-income housing program as well as drug and alcohol rehabilitation. Responsibilities:

  • Responsible for intake, assessments, service planning, service delivery, and goal attainment
  • Participates in defining procedures to enhance effectiveness and efficiencies of program operation; serves in key role for ground-level program start-up including strategic planning and program implementation
  • Manages day-to-day operations of case managers’ participant goals, objectives and treatment plans; maintains a limited caseload
  • Conducts weekly case conferencing and monthly case file audits to assure participant records are complete and accurate
  • Provides assistance and linkages for ex-offenders and maintains case load
  • Meets with clients on a regular basis to establish goals, and action steps towards those goals, and to monitor and assess progress toward achieving housing goals
  • Develops and maintains inter-agency and community relationships
  • Maintains strict confidentiality of employee, client, company, and vendor data, as well as handling sensitive and critical information with professionalism and discretion
  • Assist with health system navigation, including:
  • Help making health appointments(primary care, specialty care, behavioral health, SUD treatment)
  • Accompaniment to health appointments
  • Assisting with understanding health care
  • Assist with social services navigation, including benefits and housing, as well as social supports, employment, legal, and compliance with Probation/Parole supervision, if applicable
  • Conduct comprehensive screenings and assessments  to develop a case plan
  • Provide crisis intervention if needed, and make referrals to appropriate community resources
  • Maintain complete client records, daily activity logs, mileage logs, CHAMP and other reports as directed
  • Other duties assigned

Education:

  • Bachelor's degree or equivalent combination of education and experience

Experience:

  • 1 to 3 years experience working with homeless individuals
  • Demonstrate excellent computer skills with proficiency in Microsoft office
  • Excellent oral, written, organizational, and motivational skills

Other Preferred:

  • Must be comfortable and able to work with ex-offender population
  • Must have current valid California driver's license and clean driving record
  • Bilingual (Spanish) preferred
