Project SLATE (South Los Angeles Team for Empowerment) is an innovative initiative designed to provide a wide array of case-managed supportive services to help young fathers ages 14-24, who have been released from incarceration and assigned to the Los Angeles County Probation Department, transition to long term self-sufficiency and responsible fatherhood. All participants will receive wraparound supportive services, case management, mentoring, parenting classes and court advocacy, job readiness training, employment services, industry recognized certificate training, substance abuse treatment, housing support and benefits advocacy to assist and support their transition from incarceration back into their communities and the lives of their families.


  • Responsible for intake, assessments, service planning, and service delivery
  • Participates in defining procedures to enhance effectiveness and efficiencies of program operation;
  • Manage day-to-day operations of case managers' participant goals, objectives and individual reentry plans; maintains a limited caseload
  • Conducts case conferencing and case files audits to assure participant records are complete and accurate
  • Facilitates groups and completes required documentation; evaluates sessions for appropriateness and effectiveness
  • Collaborate with other service providers to link participants to community resources and supportive services
  • Minimize service gaps, ensure continuity of care and assist with overcoming barriers
  • Ensure completeness of participant charts and accuracy of all documentation
  • Ensure accurate data is recorded for reporting to Department of Justice
  • Other duties as assigned  


  • Bachelor's degree or equivalent education and experience in related field
  • Proficiency with Microsoft Office
  • Effective written and oral communications skills
  • Must possess a valid driver’s license and dependable transportation with adequate vehicle insurance
  • Must be comfortable and able to work with youth/young adult (ages 14-24) ex-offender population
  • Must be willing to travel 25% of the time

Additional Preferred

  • 1 - 3 years’ experience working in case management services (preferred)
