Housing Specialist - Office of Diversion and Reentry

Office of Diversion and Reentry provides mentoring both pre and post release, case management, and transitional support services.


  • Identify and locate housing options suitable for households as needed
  • Develops support systems to meet individual needs; coordinates and facilitates move-in activities, rental negotiations, and service referrals as required.
  • In conjunction with the Coordinator, computes rents based on financial information provided and established formulas and guidelines
  • Maintains working relationships with various community property management firms and property owners.
  • Connect with landlords and familiarize them with the Office of Diversion and Reentry program and community resources to assist both tenants and landlords in the event of potential problems leading to eviction
  • Responsible for creating and maintaining a housing database linking tenants to apartments
  • Will coordinate follow-through services with case management team and conduct habitability and lead based paint inspections as requested
  • As a member of the program case management team, will provide information, referrals, crisis intervention, and assistance with permanent housing placement to all participants and Case Managers
  • Provide outreach and engagement services to the literally homeless in the community, while specifically targeting the chronically homeless population.
  • Conduct the VI-SPSDAT and other comprehensive screenings and assessments with homeless individuals to collect functional, environmental, employment, housing, educational, and health information, as appropriate, to develop a case plan.
  • Develop individualized service plans with each participant to address their housing needs, as well as other identified needs.
  • Provide service linkages and support systems for individuals, to ensure identified needs are met.
  • Make referrals to supportive services such as job developers, social workers, and health care providers.
  • Provide supportive counseling and advocacy for individuals; provide crisis intervention, if needed.
  • Provide retention and follow up services to ensure housing stability.
  • Document and maintain all case files in physical files, HMIS or other identified databases.
  • Other duties as assigned.

Required Background and Skills:


  • Bachelor's degree in social work or related field or equivalent combination of education and work experience


  • 1 - 3 years' experience working with homeless families/individuals or people in recovery.
  • Ability to develop relationships with families as well as a variety of service stakeholders (schools, health and public benefit agencies.)
  • Provides a social and emotional culture that supports and builds teamwork through a solution oriented approach; works constructively through any site conflicts
  • Valid California driver's license, insurance and reliable transportation
  • Effective written and oral communication skills

Special Skills Preferred to Perform this Position:

  • Knowledge of domestic violence, poverty, and federally funded mainstream programs such as General Relief, Social Security Disability
  • Proficiency with Microsoft Office; HMIS database experience strongly preferred
  • Bilingual (Spanish) preferred
