HMIS Specialist - (ODR, Jail in Reach)

We provide a variety of social services to Los Angeles area communities such as Head Start programs, Upward Bound college prep programs, veterans’ services, homeless shelters, low-income housing program as well as drug and alcohol rehabilitation. This jail in-reach program connects chronically homeless, dual diagnosed, frequently incarcerated individuals with a permanent housing solution. Embraces a “Housing First” approach, which prioritizes quick access to permanent housing without requiring “housing readiness”. Multi agency and inter-disciplinary provides wrap-around support. Connecting the most vulnerable to housing. Housing placement, employment services, mental health services, benefit services, case management, drug and alcohol treatment.

Responsibilities:

The responsibilities are summarized as:

  • Verifies, corrects and enters participant information into centralized database; conducts regular audits to assure accuracy of data
  • Monitors database (HMIS) and (CHAMP) reports to assure consistency with other hard copy records and logs
  • Runs regular progress reports; creates customized reports to support shelter management and to provided other needed information
  • Coordinates with Case Managers and Intake staff to ensure HMIS intake requirements are met
  • Trains staff on database (HMIS) and (CHAMP) and answers questions relating to the system
  • Oversees the privacy/confidentiality notification processes to meet regulatory guidelines
  • Provides Admin and clerical support to staff when needed
  • Other duties as assigned

Requirements:

  • Bachelor's degree in social work or related field or equivalent combination of education and work experience
  • Minimum of 1 - 2  years of experience using HMIS and CHAMP
  • Strong computer skills using Microsoft Office suite as well as Internet proficiency
  • Effective written and oral communications skills
  • Must be able to pass a background check
  • Valid California driver’s license, insurance and reliable transportation

Additional Preferred:

  • At least 2 years of experience working with re-entry population, individuals experiencing homelessness, and/or special needs populations
  • Bilingual (Spanish) preferred
