SAMHSA Family Tree is a comprehensive and multi-system family approach to provide youth, transitional age youth and their families/primary caregivers with a SUD or COD who reside in LA County, assessments, evidence-based SUD/COD education, early intervention and treatment, peer recovery support, and coordination of case management and wraparound supportive services.
- Work with project staff and partnering agencies, as applicable, to develop and implement the Individual Service Plan/Family Service Plan and Aftercare Plan, which will determine age-appropriate goals, and implement the coordination of recovery and wraparound support services
- Ensure the project coordinates with other agency-based services and community-based agencies as applicable and appropriate such as health care services, mental health care and co-occurring treatment, case management, benefits advocacy, housing assistance, education and employment services, victim services, and other programs to support the successful recovery of program participants and their participating family members/caregivers (e.g. parents, spouses, children, guardians, etc.).
- Coordinate individual, group, and family peer supports utilizing best practices
- Coordinate with VOALA Benefits Specialists to enroll participants in benefits and assist as needed with collection of medical records and other assessment information, participant rights and responsibilities, services they are eligible to obtain, and transportation as needed
- Maintain an adequate caseload and practice time management skills to assure the necessary level of services is provided to participants and assist with follow-up services
- Keep abreast of new trends and advancements in the area of behavioral health and serving youth, young adults and families
- Attend staff/Project Management Team meetings as required; coordinate with evaluation team on evaluation requirements
- Ensure that provided services adhere to confidentiality and standards for quality of care according to federal and state regulations, and agency policies and procedures, and fidelity
- Other duties as required and is subject to change at any time
- Minimum of an associate’s degree (prefer bachelor’s degree) or equivalent education and experience
- Experience working youth, young adults and families, and individuals and persons experiencing behavioral health disorders
Special Skills Required to Perform this Position:
- Knowledge of participant interviewing and follow-up locating skills.
- Must have demonstrated computer proficiency, including knowledge of word processing, spreadsheet and database software.
- Experience with data collection and data entry must be self-directed and able to work with minimum supervision;
- Must be capable of making independent judgements; must be sensitive to the needs of those who have experienced trauma and victimization;
- Must have the ability to work with people from various ethnic groups in a culturally competent manner and in a trauma-informed care approach; must be able to communicate effectively, verbally and in wring with professional and participants; bilingual a plus.
- Travel to service sites involved in project implementation required
