JOB SUMMARY AND PURPOSE
The Community Health Worker II (CHW) provides culturally appropriate health education and information, helps people get the care they need, gives informal counseling and guidance on safety, social distancing and health behaviors, advocates for individual and community health needs.
DUTIES AND RESPONSIBILITIES
Outreach and community mobilization
- Prepare and disseminate material, find and recruit participants, assess community needs, promote health literacy, be an advocate.
- Organize communities, be an advocate, translate and interpret, conduct assessments.
Care management and care coordination
- In coordination with Program Management and/or Medical Provider, establish the comprehensive needs of the participants.
- Assess the impact of social determinants of health on the client’s healthcare needs and develop a comprehensive health care plan.
- Facilitate client’s in their comprehensive health self-screenings to collect functional, environmental, psycho-social, employment, housing, educational, and health information, as appropriate as it relates to health outcomes.
- Conduct individualized planning sessions with client’s to meet short- and long-term needs.
- Provide service linkages and support systems to ensure identified health needs are met.
- Engage families, conduct assessments, address basic needs, set goals and plan actions, suggest referrals, give feedback to providers, produce documentation.
- Coordinate health case conferences with client’s, consults with the case management team, monitors progress of client’s on health care plan, assists client’s to meet the agreed upon goals.
- Conducts discharge and transition planning; drafts discharge summary.
- Monitor and record client’s self-administration of prescribed medication.
- Document client case management information in the EHR (training provided) and other software after client assessment or interview in a timely manner.
Health promotion and health coaching
- Teach and model behavior change, coach on problem solving, reduce harm, promote treatment adherence, facilitate support groups.
- Work collaboratively with Motivational Interviewing (training provided) approach to develop comprehensive health services plan with each client that includes a broad range of community medical/health services including assisting clients with physical and emotional needs, and referral to appropriate service providers, maintains individual client health data and information in a secure, HIPAA compliant manner.
- Help client navigate, coordinate, and follow up.
- Provide health administration services including: direct communication with medical providers, referrals to other providers and community health resources, general health instruction to clients, and instruction and in-servicing to other program staff.
- Expand system referral resources.
Outcomes Evaluation Support
- Provide input and feedback for survey development, administer surveys, and support Evaluation Department efforts.
- Be an advocate, engage partners, interview, perform data entry, and conduct web searches.
- Participates in weekly health case review with Program Staff to ensure best quality care for clients, and continuous improvement of services.
- Act as liaison between Program Staff and Medical Providers, in order to coordinate best care of client’s
- Interact with other program CHWs to develop best practices.
- Must be able to pass a fingerprint clearance, background check, including criminal history, personal references, employment and education verifications
- A total of 3 years relevant experience in case management and/or community health work
- OR Bachelor’s degree (in a healthcare or Social Services related discipline)
- OR Associate’s degree (in a healthcare or Social Services related discipline) plus two years relevant experience in Case Management/Community Health Work;
- AND Valid Cardio Pulmonary Resuscitation (CPR) certification
- Completion of CHW training within 90 days of hire
- Personal experience in dealing with under-served populations, homelessness.
- Broad knowledge of available health care services.
- Understanding of health recommendations.
- Understanding of social distancing and safety protocols
- Microsoft operating system navigation: Outlook, Word, Excel, PowerPoint.
- Familiar with Electronic Health Records

- Share accurate and culturally appropriate information with clients, families, and community groups; use plain language; assess understanding; select appropriate written and visual materials.

- Create work schedule, prioritize tasks, clearly document work, and report to supervisor or work team.
- Capacity-building skills
- Identify and support community leaders, develop personal leadership skills, collaboratively organize effective community advocacy projects.

- Must be able to work flexible schedule (evenings and weekends required)

- Compassionate, caring individual of providing, including in times of stress; comfortable with addressing and correcting inappropriate behavior; able to de-escalate conflict
- Bachelor’s Degree (in a healthcare or Social Services related discipline
- Familiarity with Los Angeles County health and social services
- Bilingual in Spanish and English
- Learn and apply principles of client-centered, strength-based counselling (including the principles of the recovery model and trauma-informed practice)
