Our SSVF Program (Supportive Services for Veteran Families) provides temporary housing assistance in conjunction with comprehensive case management to veterans with families who are homeless or are facing a housing crisis.

Responsibilities:

The SSVF-OC Case Manager will arrange, monitor, and deliver services related to meeting the needs of SSVF program veteran households while helping them obtain housing stability. The SSVF-OC Case Manager duties are summarized as:

  • Manages a caseload of clients; maintains case files
  • Conducts comprehensive client assessments to collect functional, environmental, psychosocial, financial, employment, housing, educational, and health information as appropriate to develop a case plan
  • Develops support systems to meet client needs by identifying and coordinating a variety of available services necessary to maintain independent living, and, when possible, self-sufficiency and family stabilization
  • Collects necessary documentation for SSVF verification purposes
  • Provides supportive counseling and advocacy for clients
  • Refers veteran household to available VA/Community providers for therapeutic, social, educational, and medical resources when appropriate
  • Conducts crisis intervention as necessary
  • Monitors and verifies services provided to each veteran household on a regular basis, determining quality and effectiveness of services provided
  • Conducts new veteran household intakes and ensures all supporting documents are verified/filed
  • Interviews veteran household for emergency assistance and determine eligibility
  • Documents and maintains case management files for all veteran households
  • Monitors and documents progress of veteran households
  • Administers direct assistance of funds by way of third party check payment
  • Provides information and referrals for callers and walk-in veteran households
  • Develops, performs, and assists with workshops pertaining to veteran households needs
  • Participates and assists with outreach events
  • Other duties as assigned

Requirements:

  • Bachelor's degree from an accredited college or university preferred
  • 1 - 3 years experience working as a Case Manager with veterans, people in recovery and/or homeless individuals
  • Demonstrated proficiency in MS Word, Excel, and internet usage. HMIS a plus
  • Ability to develop relationships with families as well as a wide variety of service stakeholders (schools, health and public benefit agencies)
  • Ability to lift up to 25 pounds

Additional Requirements:

  • US Armed Forces veteran preferred
  • Bilingual (Spanish) preferred
  • Valid California driver’s license, insurance and reliable transportation
