Case Manager Specialist - (Pathways to Home)

Pathways to Home residents begin a journey toward a permanent, sustainable housing situation from the moment they walk through the door. Each resident has an action plan and the support services they need to find work, get access to benefits they may be eligible for, get access to healthcare and ultimately get access to long term housing.

Responsibilities:

The Case Manager Specialist:

  • Active case load management through weekly/bi-monthly individual meetings led by individual service plan action items
  • Maintain and develop referral network of preferred providers
  • Adaptability to contractual and policy standards
  • Ability to perform bard and soft file management, HMIS, CM process flow. Clinical documentation-OAP note proficiency, and caseload management.
  • Understanding, ability to assess, and consult with team on participant acutiy and appropriate levels of care
  • through treatment team meetings
  • Maintain an ethical standard of care though participant first model and continuing education of HIPPA
  • applications
  • Client engagement strategy and education to perform quality assessments
  • Small group facilitation
  • Other duties as assigned  

Requirements

Education:

  • Bachelor’s Degree (Social Work or related field) OR equivalent related job experience (6 years)

Experience:

  • At least 1 year of experience in case management in re-entry, housing, benefits or job development (2 years required if no degree)
  • At least one year of experience with the target population

Knowledge:

  • Case Management ethics, HIPAA, Programmatic standards of operation and clinical engagement, clinical documentation methods, client focused ideology

Skills:

  • Able to oversee the work of case management staff and/or monitor staff, as assigned
  • Strong skills in interpersonal relationships
  • Ability to lead others and provide training
  • Ability to troubleshoot complex problems and find specialized solutions when needed

Preferred:

  • Master’s Degree (Social Work or related)
  • 2 years of experience in case management and working with target population
  • Experience in supervision or lead role preferred
  • HMIS navigation
