Case Manager (Southwest 2 GRYD)

24-Hour incident response coverage in and around the GRYD Zone in an effort to reduce gang violence and engage families impacted by gang violence

Responsibilities:

  • Serves and engages gang-involved youth or young adults, ages of 14 to 25 years in a GRYD Zone
  • Strategically plan and work with community to decrease and eliminate gang activity and violence
  • Plans strategies to engage local gangs/crews proactively maintain peace via safe passages, and activities
  • Work with Law enforcement to create peace in the community
  • Maintain a minimum of 22 FCM clients updated and active
  • Maintain a minimum of 10 TCS client updated and active
  • Audit weekly TCS files of Community Intervention workers
  • Data entry of Entry to Outcomes (ETO) on client’s activities and progress
  • Follow all phases of case management and meet with case manager weekly to audit files
  • Maintain weekly contact with schools, community based organizations, gangs in the area
  • Meet weekly with all case managers
  • Report weekly on progress of case management
  • Apply Social Embeddedness Tool (SET) to youth as required.
  • Run weekly reports to maintain data base error free and up to date
  • Participate in planning, meetings and work at Summer Night Lights (SNL) and FFN sites
  • Make home visits to assist clients and family with services needed
  • Accompany youth to probation appointments and submit monthly probation progress reports.
  • Build a relationship with all ethnicities, genders, etc.
  • Work with multiple gangs in the area
  • Facilitate workshops Ex. life skills, substance abuse, etc.
  • Do street outreach and pass out literature and services
  • Create monthly calendar of activities
  • Create monthly community engagement, parent and client activities
  • Travel and drive for program outings and activities
  • Transport clients as needed
  • Keep track of community service workers
  • Adapt to new and changing situations in program
  • Manage time very well
  • Attend monthly meetings
  • Support all staff in community and in-house activities
  • Other duties as assigned

Requirements:

  • 4 years' experience working with At-risk families/individuals or gang population
  • Proven ability to interact positively with diverse groups of client population with varying education and economic backgrounds particularly gang affiliated individuals
  • Proficiency with Microsoft Office, including Excel; database experience
  • Excellent written and oral communications skills
  • Work late, weekends and holidays (flexible schedule)
  • Valid California driver’s license, insurance and reliable transportation
  • Able to have integrity and work independently without supervision

Additional Preferred:

  • Bilingual (Spanish preferred)
  • LAVITA certification preferred but not required
  • Trained on the GRYD Model of practice and the efforts to Outcome Database
