Case Manager - (Project SLATE)

Project SLATE (South Los Angeles Team for Empowerment) is an innovative initiative designed to provide a wide array of case-managed supportive services to help young fathers ages 14-24, who have been released from incarceration and assigned to the Los Angeles County Probation Department, transition to long term self-sufficiency and responsible fatherhood.  All participants will receive wraparound supportive services, case management, , mentoring, parenting classes and court advocacy, job readiness training, employment services, industry recognized certificate training, substance abuse treatment, housing support and benefits advocacy to assist and support their transition from incarceration back into their communities and the lives of their families.


  • Identify needs, goals and objectives of participant
  • Develop Individual Reentry Action Plan (IRAP)
  • Complete participant assessments
  • Document accurate and concise progress of participant
  • Provide ongoing support, assistance and motivation to Project Slate participants and staff
  • Conduct weekly case management meetings with Project Slate participants to develop, plan and track progress towards goals
  • Match and coordinate mentors with participants
  • Assist in conflict mediation and problem-solving as needed
  • Create and maintain relationships with outside agencies that serve as advocates and resources for Project SLATE  participants
  • Maintain ongoing contact with past participants and provide additional follow up as needed
  • Maintain participant files and record outcomes
  • Participate in weekly case management and staff meetings.
  • Establishes and maintains relationships with community partners
  • Assesses potential participants for level of ability and readiness for program engagement
  • Monthly case file audits
  • Weekly case conferencing
  • Other duties as assigned

Required Background and Skills:


  • Bachelor's degree or equivalent combination of education and experience


  • 1 - 3 years experience working as a case management services (preferred)
  • Proficiency with Microsoft Office
  • Effective written and oral communications skills

Other Requirements:

  • Must be comfortable and able to work with ex-offender population
  • Must be willing to travel 25% of the time
  • Valid California driver's license, dependable transportation, and adequate vehicle insurance
