Case Manager - Pomona Hope for Homes Services Center

We provide a variety of social services to Los Angeles area communities such as Head Start programs, Upward Bound college prep programs, veterans’ services, homeless shelters, low-income housing program as well as drug and alcohol rehabilitation. Responsibilities:

  • Conducts comprehensive screenings and assessments to collect information needed to develop a case plan.
  • Link clients to mainstream benefits and supportive services for housing, health care, domestic violence.
  • Provides assistance and linkages for homeless individuals and maintains case load.
  • Develops goals and timelines for service delivery in accordance with requirements
  • Maintains documentation for caseload of 25 or more clients with respect to activities and progress to goal satisfaction
  • Completes weekly case conferencing with Program Manager, and maintains complete and accurate documentation of services in accordance with federal, state, county, and VOALA guidelines
  • Meets with clients on a regular basis to establish goals, and action steps towards those goals, and to monitor and assess progress toward achieving housing goals
  • Develops and maintains inter-agency and community relationships with providers of supportive services within and outside of VOALA
  • Maintains strict confidentiality of employee, client, company, and vendor data, as well as handling sensitive and critical information with professionalism and discretion
  • Other duties as assigned

Requirements:

Education:

  • Bachelor's degree in social work or related field preferred or equivalent work experience in lieu of degree

Experience:

  • Minimum 1 year experience working with homelessness or other related social services
  • Ability to develop relationships with families as well as a variety of service stakeholders and community providers (schools, health and public benefit agencies.)
  • Excellent oral, written, organizational, and motivational skills
  • Provides a social and emotional culture that supports and builds teamwork through a solution oriented approach; works constructively through any site conflicts

Special Skills Required to Perform this Position:

  • Knowledge and experience working with public housing authority voucher and flexible subsidy pool programs
  • Knowledge of domestic violence, poverty, and federally funded mainstream programs such as General Relief, Social Security Disability
  • Proficiency with Microsoft Office
  • Effective written and oral communication skills
  • Valid California driver's license, insurance and reliable transportation
  • Successful completion of tuberculosis and background screening

Preferred Skills for this Position:

  • HMIS database experience strongly preferred
  • Proficiency with Microsoft Office
  • Bilingual (Spanish) preferred
