Care Manager Generalist (ODR)

We provide a variety of social services to Los Angeles area communities such as Head Start programs, Upward Bound college prep programs, veterans’ services, homeless shelters, low-income housing program as well as drug and alcohol rehabilitation. In Diversion and Re-Entry, we evaluate and refer clients to appropriate service modalities where they will receive mental health services and access to various benefits, as well as housing and supportive services. Services include, crisis intervention, case management, transitional housing, and life skills counseling in a field setting. Program participants will have will be involved with criminal justice system as well as suffering from co-occurring disorders.

This position provides clients with the highest levels of acuity and co-morbidities individualized care through access to service linkages, counseling, treatment planning, and various crisis intervention methods in order to prepare individuals for further success.  

Responsibilities:

  • Manage high acuity and co-morbidities case load through individual meetings on a regular basis and develop a detailed plan of action to meet the high acuitity clients needs, set goals and find the necessary resources to meet those goals
  • Conducts comprehensive screenings and assessments to collect information needed to develop complex case plans
  • Provides high-level crisis management services by communicating regularly with clients
  • Conducts follow up meetings to track program progress
  • Facilitates groups and completes appropriate documentation on all aspects of participant case
  • Deep understanding, ability to assess, and consult with team on participant acuity and appropriate levels of care through treatment team meetings
  • Maintains confidentiality by following HIPAA regulations, respect clients privacy and follow the Participant First Model
  • Keep comprehensive records of clients progress by performing hard and soft file management, HMIS, CM process flow, clinical documentation – DAP note proficiency, and caseload management
  • Responsible for appearing in court with clients (before judges) to manage their cases and re-entry into society. Serves as advocate on the part of the clients served
  • Maintains documentation for caseload of clients with respect to activities and progress to goal satisfaction
  • Collaborates with other service providers to link participants to community resources and supportive services
  • Other duties as required and is subject to change at any time

Requirements

  • Education
    • Bachelor Degree (Social Work or related), required OR equivalent job experience (6 years) OR combination of education and experience
  • Clearances
    • Must be able to pass Los Angeles Sherriff Department (LASD) background check
    • Valid California driver's license and have reliable transportation
    • Approval from Department of Health Services (DHS)
    • TB Test
  • Experience
    • Minimum 1 year experience in case management in homelessness, additions, under-served and/pr prison re-entry program
  • Knowledge
    • Case Management ethics, HIPAA, Programmatic standards of operation and clinical engagement, clinical documentation methods, client focused ideology
    • Knowledge of and ability to work with issues related to homelessness, aging in place, substance abuse, mental health diagnosis, and history of trauma
  • Skills and Abilities
    • Basic Microsoft operating system navigation 
    • Strong written and verbal communication and presentation skills
    • Able to handle multiple tasks
    • Ability to problem solve
