Job Description

Position Purpose:

The social worker in this position is responsible for an assigned case load in a residential center providing nursing home, domiciliary, and personal levels of care. Primary duties include: completing admission and discharge procedures, providing ongoing casework and medically related services, working with an interdisciplinary team to identify and meet the needs of the residents, maintaining required documentation, and knowing and complying with the requirements and standard of regulating bodies, i.e., Health Department, DHS, and the VA.

Licenses, registrations, or certifications:

  • Licensed Social Worker with a Master of Social Work is required.

Description of Duties:

10% Serves as coordinator for behavioral and psychosocial intervention services with the health care community. In this capacity, the employee assesses, designs, and implements appropriate behavioral and psychosocial interventions for residents deemed in need of this specialized service. Offers consultation and education to other disciplines in developing behavioral management programs. Serves as the consultant to other social workers in assisting their management of resident problems due to behavioral or psychosocial issues. As part of this consultation, will prioritize and assign specific tasks for social workers to carry out and comply with federal guidelines related to serving veterans with dementia.

10% Provides assistance to the director in maintaining the effectiveness of the department by completing a post-admission survey of family members, staff and various health care providers, analyzing the data, and making recommendations for changes in determination of facilities capability to meet the needs of residents.

5% Serves as the director's designee on the facility's resident abuse investigation team.

10% Meets with new residents and responsible parties on admission to orient and to complete admission documents. 10% Provides direct social work interventions for all residents and families on assigned units which can include counseling, referrals to other departments and outside agencies, notification of changes, periodic review of needs, and discharge planning.

5% Completes assigned section of the resident assessment instrument for all assigned residents on a quarterly, annual and as needed basis.

5% Attends and actively participates in assigned team meetings. Intervenes on all appropriate interdisciplinary care plans. 10% Completes quarterly psychosocial assessments on each assigned resident according to prescribed format. Updates assessments according to prescribed format. Records on progress record to meet SEVC and Social Service Department requirements.

5% Serves as advocate for residents who believe their rights have been violated. Provides ongoing education and assistance to residents and families regarding rights and advance directives.

5% Attends required meetings and in-service training.

5% Provides director with on-going information about concerns, problems, and needs of assigned units and residents. Engages in supervisory sessions as requested.

5% Provides pre-admission information and tours to applicants and prospective applicants as needed.

5% Support the facility QAPI efforts by attending all assigned QAPI training; following established policies and procedures in support of QAPI efforts; participate in PIP teams as assigned; sharing ideas for improvement in the organization; and supporting QAPI efforts both verbally and non-verbally, including adjusting performance and practice in accordance with QAPI initiatives and findings.

5% Performs work in accordance with the DMVA general safety rules and safety rules for this position. Immediately reports all workplace injuries or incidents to the supervisor; and/or immediately corrects any safety hazards in the work area or reports same to supervisor.

Use personal protective equipment when required such as gloves, masks, face shield, etc.

5% Performs other related duties as required.

Decision Making:

Makes decisions regarding clinical issues managed the Social Services Department. These areas are identified in the job description and specifically in residents' care plans. Clinical issues that are extremely complex, highly volatile, or highly sensitive will need involvement and final approval from the supervisor. This position also provides decision-making for the department when the supervisor is absent or unavailable.
