Minimum Qualifications:
- Bachelor of Social Work from an Accredited Social Work Program or equivalent education and experience.
- Basic computer skills to include, but not limited to, Microsoft Office Products.
MAJOR POSITION DUTIES AND RESPONSIBILITIES:
- In conjunction with the other regional monitoring team members, perform on-site and off-site monitoring reviews for OLTL-HCBS enrolled providers.
- Review providers' written documentation specifically to ensure participants are receiving appropriate care and that the services being provided are following all program requirements.
- Review written documentation and meet with providers, participants and stakeholders to evaluate the quality of services being provided.
- Remain alert to indications of abuse, neglect and exploitation issues, and be aware of the roles and responsibilities of HCBS providers and reporting requirements.
- Assist in the gathering of necessary background data related to participant care to determine what the providers' strengths and weaknesses have been in the past and tailor monitoring to target specific needs for each provider.
- Interview provider staff, stakeholders, and participants as part of the monitoring proces
- Perform research through direct contact or by electronic means with provider staff, participants, and stakeholders.
- Assist in the preparation of monitoring tools and objectives for provider reviews.
- Assist in changes and upgrades to data collection tools.
- Organize and compile monitoring data into a cohesive quality management report for each provider entity.
- Analyze a variety of material and data and develop conclusions.
- Assess whether the provider entity has complied with applicable regulations and guidelines.
- Successfully communicate findings related to monitoring reviews of HCBS providers.
- Research necessary information, applicable laws, regulations, and policies governing the HCBS provider entity.
- Participate in conferences, meetings, and trainings with HCBS provider entities.
