Job Description

MINIMUM QUALIFICATIONS

  • Preferred – One year of experience in case management, protective services, or an equivalent combination of experience and training
  • Two years of professional experience in a human services program providing social services to clients; and a bachelor's degree in the social or behavioral sciences, public health, or a related field; or
  • An equivalent combination of experience and training, which includes two years of professional experience in a human services program providing social services to clients.
  • A report of criminal history from the State Police and/or FBI must contain no information.

DUTIES

Respond, either verbally or in writing, to select inquiries from intra and inter-departmental units, outside entities, and adults/family/collaterals.

Provide input on draft policy and procedure May be Involved in incident reviews, closing incidents, reports, and investigations.

Assist in tracking information, reviewing reports, and preparing reports for the supervisor, and others when needed.

Maintain caseload of APS cases in which the APS vendor has reduced risk, however, require on-going long-term funding, and cases that require intensive case work.

Case investigation duties:

  • Review previously initiated adult protective services cases and perform investigative activities to complete the investigation. This may include, but not limited to, making telephone calls to collateral contacts such as family members, alleged perpetrators, physicians, banks, neighbors, friends, eyewitnesses, care providers, and other individuals or entities. Work must be completed in accordance with protective services legislation, regulations, and operating policy and procedures of the APS. Comply with legal and statutory time frames.

o Identify any unmet needs or potential risks to the adult.

o If any unmet needs are identified or any potential risks identified, refer the case back to the APS Vendor for services (preparation and implementation of an APS service plan).

o Research and coordinate across program offices to remove barriers and identify all potential services and options as needed for cases identified as having complex needs.

o If no unmet needs or potential risks are identified, close the case record in accordance with the APS database manual.

  • Utilize the professional services of agency-based medical professionals to assist with case-related assessments when necessary.
  • Document all case-related actions and results (i.e. interviews with collateral contacts, review of medical or financial records, obtainment of a Power of Attorney document, etc.) and enter all required information into the PA Department of Aging’s standardized documentation system
  • Other related duties as assigned.


ESSENTIAL FUNCTIONS

Safeguard the confidentiality of all information both electronically and in writing.

Utilize a variety of computer software programs.

Successfully complete training as directed by the PA Department of Human Services to enhance overall job performance.

Apply protective services legislation, rules, regulations, and operating procedures of the APS.

Accurately and timely record information and meet legal and statutory time frames.

Excellent verbal and written communication skills.

Strong critical thinking and problem-solving skills

Perform other related work as directed.
