Minimum Qualifications

MINIMUM EXPERIENCE AND TRAINING:

A bachelor's degree is preferred, but not required. A combination of education, experience and training, which may include experience working in a public or private human service program with at least 12 college level credits of education in a social science or human services, education, administrative experience such as case management, service delivery, data collection, or program monitoring/quality assurance experience will be considered in lieu of a bachelor’s degree Intermediate Computer skills to include, but not limited, to Microsoft Office Products.

Role Description:

Job Duties

· Monitors and provides consultative services to Supports Coordinators and providers for the Adult Autism Waiver (AAW) to assure compliance with state regulations, policies and procedures

· Evaluates the quality of AAW services and supports through on site visits and review of records

· Interviews and meets with AAW consumers to ensure needs are being addressed

· Conducts annual quality assurance monitoring of AAW providers and participates in the development, coordination and implementation of corrective action plans

· Reviews, approves and monitors Individual Support Plans (ISP)

· Provides general case management services for AAW participants

· Makes recommendations on how to improve programs and service delivery

· Determines final non-financial eligibility for the AAW

· Meets with service providers to explain waiver services, policies, and regulations and also work with providers to ensure compliance with the AAW
