Role Description:
The Pennsylvania Department of Health is committed to supporting long-term care facilities (skilled nursing, personal care, assisted living, intermediate care)in outbreak response efforts to SARS-CoV-2 and other pathogens as well as through strategic investment in and implementation of initiatives that will enable facilities to build enduring resilience for all hazards. The Department’s Long-Term Care Transformation Office (LTCTO) has launched two significant programs (Quality Investment Pilot, LTC-RISE) that directly benefit facilities towards outbreak support and quality improvement that focus on the areas of workforce development and satisfaction, infection prevention and control, and emergency preparedness. The Pennsylvania Department of Health is seeking a full-time Outreach and Engagement Lead to join a team of highly motivated professionals to work directly with government agencies, health systems, and facilities to improve resident care and worker satisfaction to transform the long-term care industry towards self-sufficiency during routine times and public health emergencies. This position is considered fully remote, however onsite facility visits and in person Departmental meetings are expected periodically and as necessary to complete the mission.
The Long-Term Care Outreach and Engagement Lead will play acritical role within the Pennsylvania Department of Health’s Long Term Care Transformation Office, leading efforts to enhance quality and build resiliency in long term care (LTC) facilities across the state. This position requires a dedicated professional with a passion for public health, long term care, and improving outcomes for aging populations. The Outreach and Engagement Lead will serve as a key point of contact between the Department of Health and LTC facilities, fostering strong relationships, providing guidance on LTCTO initiatives, and coordinating programs that promote high-quality, sustainable care practices.
Key Responsibilities:
Outreach and Engagement:
Build and maintain relationships with LTC facility administrators, healthcare providers, and community organizations.
Serve as a liaison to connect LTC facilities with resources, training, and technical assistance that enhance quality of care and operational resiliency.
Facilitate regular communication between LTC facilities and the LTCTO to share updates on best practices, regulatory changes, and innovative care models.
Program Implementation and Support:
Lead the implementation of quality improvement initiatives within LTC facilities, ensuring alignment with state and federal standards.
Collaborate with other public health officials, policymakers, and stakeholders to identify opportunities for advancing care quality and addressing emerging challenges in LTC.
Monitor the progress of initiatives aimed at enhancing infection control, staff training, emergency preparedness, and overall resident care quality.
Education and Training:
Develop and deliver training programs focused on building resiliency in LTC facilities, including crisis management, infection prevention, and person-centered care approaches.
Assist in the creation and assessment of educational materials for LTC staff, residents, and families, with a focus on fostering a safe and supportive care environment.
Data Collection and Analysis:
Support data collection efforts related to quality improvement and resiliency measures in LTC, analyzing trends and outcomes to inform policy decisions.
Provide feedback and recommendations based on data to improve facility performance and compliance with health regulations.
Regulatory Guidance:
Assist LTC facilities in understanding and complying with state and federal regulations, including those related to quality assurance, infection control, and staffing.
Offer expertise on best practices to help facilities meet the requirements of quality reporting programs and other health-related mandates.
Qualifications:
Bachelor’s degree in public health, health administration, gerontology, or a related field (Master’s preferred).
3-5 years of experience in long term care, public health, or a related healthcare field.
Strong knowledge of long-term care operations, quality improvement processes, and state and federal regulations.
Excellent communication and relationship-building skills, with the ability to engage and collaborate with a wide range of stakeholders.
Ability to analyze data and provide actionable insights to improve care quality and resiliency.
Experience in delivering training and educational programs is preferred.
Passion for advocating for the elderly and individuals with long term care needs.
Work Environment:
This position will involve both office-based and field work. The Outreach and Engagement Lead must be comfortable traveling to LTC facilities across Pennsylvania to provide on-site support and engagement; 50% travel to LTC facilities for in person interactions. Pennsylvania Residency required.
