Job Description

Proposed Description: This position is assigned to the Department of Human Services as an assistant to both the Director of the Money Follows the Person (MFP) program and Executive Housing Director. The MFP program is implemented through a coordinated cross-system program involving DHS’s following program offices; the Office of Developmental Programs (ODP), Office of Mental Health and Substance Abuse, (OMHSAS) and the Office of Long-Term Living (OLTL). Because accessible and available housing is a main barrier for individuals transitioning from a long-term care facility and MFP funds are used for housing initiatives, the position will also work with the Department’s Executive Housing Coordinator to meet the Housing Goals for the MFP program by enhance existing OLTL partnerships to reduce barriers to housing with the PA Housing and Finance Agency (PHFA), the Department of Community and Economic Development (DCED) and local housing authorities. Initiatives include, but are not limited to, the Tenant Based Rental Assistance Program for individuals leaving nursing facilities, 811 Housing Assistance, working with the Regional Housing Coordinators and the Home Modifications and Assistive Technology programs

Proposed Duties:

  • Responsible for carrying out a wide variety of staff assignments to meet the goals and objectives of the Money Follows the Person program and DHS’s housing activities.
  • Collect and organize data that support MFP activities within the program office’s operations. Data may involve reviewing information for the MFP budget, Semi-Annual reports, Maintenance of Effort reports, relevant housing programs, initiatives, and policies as well as ad hoc requests.
  • Ensure data accuracy and consistency between the identified populations, the MFP Operational Protocols and CMS requirements.
  • Prepares reports that reflect program office activity and progress toward meeting the goals and objectives of the MFP program.
  • Prepares tables and graphs from analysis for inclusion in work products which may be presented orally, in memorandum form, or in the form of narrative statistical reports.
  • Provides guidance and technical assistance to departmental, county, and health and human services providers.
  • Reviews and recommends modifications to program manuals and materials based on legislative, regulatory or policy revisions.
  • Track eligible individuals for enrollment in MFP, by utilizing the Client Information System (CIS).
  • Develop written and operational procedures, to streamline the MFP eligibility process, communication with all involved parties, and timely and accurate identification, to support accurate MFP reporting.
  • Undertake and complete research assignments such as researching current national trends and innovations in HCBS and housing options.
  • Arrange meetings and conference calls as directed.
  • Utilize the 811-housing reporting system to include data collection, review and analysis,
  • Help develop written materials and resources to assist reporting, verification, auditing, and project activities.
  • Assists in designing and revising presentation materials based on information provided by the MFP Director and Executive Housing Coordinator for training and program reporting purposes.
  • Represent or accompany the MFP Director and Executive Housing Coordinator at meetings, workgroups, and committee follow-ups when needed.
  • Participate in relevant housing and MFP executive staff meetings.
  • Remain up to date on changes to federal and state policies and regulations related to the MFP Program and PA’s Housing Assistance programs that may result in changes to MFP operations, policies, and reporting for compliance.
  • Perform other duties as assigned.

Required Knowledge, Skills, and Abilities:

  • Knowledge of human services or health care programs
  • Knowledge of Pennsylvania’s housing assistance programs and activities.
  • Knowledge of the principles and methods applied to collecting, reviewing, analyzing, and interpreting program and statistical data Ability to learn agency programs, projects, and operations.
  • Ability to analyze and evaluate program plans and operations.
  • Ability to make recommendations regarding policy development and implementation.
  • Ability to coordinate the development of departmental program plans.
  • Ability to monitor and evaluate corrective action plans.
  • Ability to provide training, technical assistance, and consultation for program issues to relevant parties.
  • Ability to establish and maintain effective working relationships.
  • Ability to communicate both orally and in writing.
  • Ability to work independently.
  • Ability to use spreadsheets, word processing, and database software for special projects and tracking systems with minimal direction.
  • Ability to be flexible and organize around shifting priorities, to achieve short- and long-term goals.
