Job Description

Minimum Qualifications:

  • One (1) year of professional experience in developing, implementing or evaluating human services, sociology, public health, health care services, or health care insurance programs; and a bachelor’s degree in Human Services or Education Field

OR

  • Any equivalent combination of experience and training that includes at least one

(1) year of professional experience in developing, implementing or evaluating human services, education, sociology, public health, health care services, or health care insurance programs.

  • Resource must have knowledge of available resources to low-income residents
  • Intermediate Computer skills to include, but not limited, to Microsoft Office Suite.
  • Technical computer software skills are required.
  • Possess the ability to work as part of a workgroup or team is required for the unit’s assigned projects.
  • General research and on-line computer research skills are required.
  • Resources also needs to understand and be familiar with the operation of human service governmental operations at the federal, state, county and local levels.
  • Project troubleshooting and problem-solving skills are required.
  • Resource must have communications skills and also possess complex, technical analysis and interpretive skills.
  • Resource will be required to implement the necessary controls and procedures to ensure that responsibilities are performed to meet objectives.

Role Description:

The Community Resource Coordinator serves as a liaison with local community organizations to provide support to licensed personal care homes serving low-income populations.

Must reside and be able to travel to the following locations: Armstrong, Clearfield, Forest, McKean, Beaver, Crawford, Indiana, Mercer, Butler, Elk, Jefferson, Venango, Clarion, Erie, Lawrence, Warren, and Westmoreland

Job Duties and Responsibilities:

  • Serves as a liaison to local agencies, professional boards, task force groups, churches and non-profit organizations. Collaborates with other agencies to develop and coordinate resources in order to establish effective working relationships; receives and make appropriate referrals. Monitors and analyzes the effectiveness of service resource delivery within department; maintains a database of resource providers and services; prepares reports as needed.
  • Conduct on-site assessments of need within facilities to identify most appropriate resources and provide technical assistance to facilities to access available resources to residents and facilities such as applying for available funding, utilizing food banks, applying for eligible benefits.
  • Engage in problem-solving activities with facilities to identify solutions to emerging situations.
  • Attend regional Risk Management meetings to identify areas of concern, share resources and advocate for additional shared resources.
  • Provides presentations and training to licensed facilities, Dept of Human Services staff and local community organizations such as county MH/ID programs, Area Agencies for Aging, local Ombudsmen, etc.
  • Provides a written report of monthly activities to the Director of Operations and Regional Director.
  • Coordinates with regional directors and participates in staff meetings to identify facilities that may be at risk.
  • Other related duties as required.

Essential Requirement:

  • This position will be in the Department of Human Service’s OLTL Bureau of Human Services, Division of Training and Communication, located in Harrisburg, PA.
  • This position is a teleworking position. You must have a securely configured high-speed internet connection.
  • Travel: Regular travel within the Area to attend meetings, liaison with resources and conduct on-site visits.

Work Schedule: The standard hours of work are 37.5 hours a week.
