Position Title: Adult Day Nurse LPN/RN

Part Time / Occasional

The Gathering Place is seeking an Adult Day Nurse LPN/RN to join our person-centered care team. The LPN/RN Nurse will provide services to participants and families within the scope of practice of the LPN/RN license as defined by the State of Vermont Board of Nursing.

Essential Functions

  • Supports the mission and values of The Gathering Place

  • Interacts with participants, staff, family care givers, and the general public in ways that demonstrate TGP’s commitment to courtesy, service, and hospitality

  • Responsible for participant health assessments, reassessments and discharge plans

  • Along with participants, families and/or program staff, responsible for the development, maintenance and documentation of individual goal plans

  • Responsible for routine and preventive nursing care, including: health monitoring; medication administration; medical crisis management; communication with physicians and families; vital sign checks; health nutrition and medication education

  • Efficiently gathers information from participants and/or caregivers in person or on the phone recognizing their limits of knowledge and experience and plans for management of situations beyond their expertise, using provider and RN available resources  

  • Responsible for the accurate and timely documentation of services

  • Responsible for medication documentation and inventory

  • Responsible for the referral needs of participants, their families and caregivers and coordinates the scheduling of therapies

  • Responsible for compliance with industry and professional standards of nursing services

  • Ability to lift equipment and transfer participants

  • Responsible for education and support of the caregivers

  • Coordinates specific work tasks with department staff as well as with other departments in order to ensure the smooth and efficient flow of care and information

  • Some supervision of direct care staff and new hire and annual verification of competencies

  • Medication Passed

  • Maintain medication sheets with the Physicians order placed in each participant’s chart and medication book. A photo of each Participant scheduled for medication needs to be placed in the medication book.

  • Get all monthly flow sheets, DHRS- Monthly flow sheet and medication sheets ready for the end of the month change over.

  • Check all medications and supplies for expiration dates. Review and list supplies that we need or need replacing. Ordering is completed before the 15th of each month and place orders on the 15th or next business day after 15th.

  • Check all transmitter weekly and chart on the transmitter log each week.

  • Assist with filing the end of the month Medication sheets, DHRS- forms, Monthly Flow Sheets into Participant charts.

  • Interacts with all colleagues in ways that assist one another in the successful performance of their duties and creates an atmosphere that promotes a positive and respectful work environment; works collegially with the Management Team to proactively problem solve issues
  • Provides support, encouragement, and direction to the individuals under his/her direct supervision; acts as a resource for staff in problem solving and properly communicates within the established chain of command
  • Identifies, prevents, and reports actual or suspected abuse, neglect, exploitation, and improper use of chemical and physical restraints, in accordance with standards
  • Abides by the current laws and organizational standards and procedures designed and implemented to promote an environment which is free of sexual harassment and other forms of illegal discriminatory behavior in the work place
  • Cooperates with, participates in, and supports the adherence to all TGP standards, procedures, and practices in support of risk management and overall safety and soundness and compliance with all regulatory requirements, i.e., HIPAA, OSHA, etc.; insures that the department and all personnel adhere to the same.
  • Provides periodic reports as required.
  • Displays appropriate knowledge of the Participant Bill of Rights.
  • Demonstrate proper procedures in responding to a fire drill, reporting employee injuries and reporting any security incidents.
  • Ensures process for participants to lodge a complaint is communicated in a clear manner to meet TGP objectives
  • Demonstrates proper procedure for protecting confidential information, including guidelines when participant specific data is displayed on a computer screen; always maintains security of computer access codes.
  • Acts within the scope of his/her authority to practice
  • Responsible for other duties that from time-to-time may be required

Education, Experience, Certifications

  • Licensed Practical Nurse with current Vermont State license

  • At least one year’s nursing experience, preferable working with elders and adults with disabilities and their families

  • CPR certificate
