Southern California Alcohol and Drug Programs, Inc. (SCADP) was established in 1972 in a grassroots effort by community members seeking alternatives to prison for individuals with substance use disorder (SUD). SCADP is a 501 (c) (3) non-profit organization providing residential treatment services and outpatient counseling services to individuals throughout Los Angeles. Offering services with a continuum of care to underserved and disadvantaged populations.
La Casita is a member of the Los Angeles County Drug Medi-Cal Organized Delivery System provider network. As a Certified Drug Medi-Cal Provider, the program provides residential Substance Use Disorder treatment to parenting and non-parenting women as well as pregnant and postpartum women in a residential setting.
Residential Treatment for substance-use disorder, the program ranges from 30 to 150 days depending on medical necessity and the need of a controlled environment to establish a foundation for recovery. Our gender specific programs provide an evidence-based trauma-informed care model of utilizing cognitive behavior therapy and motivational interviewing. Program emphasis includes instruction on developing coping and life skills allowing each individual to maintain a healthy, productive lifestyle and overcome the stress and barriers to successful daily living. We provide wrap-around services including assessments, individual and group therapy and counseling, treatment planning, case management and referrals. All potential beneficiaries must have or qualify for Medi-Cal or My Health LA coverage within Los Angeles County. Ideal candidates will meet the following criteria: be over 18 years of age, have an imminent need for substance-use disorder treatment, and to be comfortable in a multicultural setting with individuals of diverse ethnicities and lifestyles.
JOB SUMMARY AND PURPOSE
Case Manager, AOD II, assesses participants and coordinates access to medical or mental healthcare, courts or other community services. The Case Manager is the primary point of contact to coordinate services and care for assigned clients. In collaboration with the AOD Counselor and Therapist, the Case Manager ensures that critical linkages to support services meet the medical, mental health, and housing needs of the participant are consistently and effectively delivered.
DUTIES AND RESPONSIBILITIES
- Conducts comprehensive screenings and assessments with individuals to collect functional, environmental, psycho-social, financial, employment, housing, educational, and health information, as appropriate
- Assess barriers facing the clients and develop a case plan.
- Coordinate individualized planning with clients to meet short- and long-term needs.
- Provide service linkages and support systems to ensure identified needs are met.
- Coordinates case conferences with clients, consults with service team, monitors progress of clients on case plan, assists clients to meet the agreed upon goals.
- Communicates and corresponds for client with judicial, community, social service, and administrative bodies as needed; facilitates groups and monitors client participation
- Network and communicate with referral agencies and community resources, as well as family members and natural supports
- Responsible for transferring benefits from previous county of residence to Los Angeles County for beneficiaries who have moved
- Facilitate necessary transitions in substance abuse treatment between levels of care (from residential to outpatient treatment, etc.)
- Documents all sessions and linkage to resources/ services in EHR
- Trains and Supervises colleagues who are earning their AOD Certification (employees with current AOD Registration who are completing required supervised hour).
- Other duties as may be required to meet program goals, and the goals of program participants
- Must be able to pass a fingerprint clearance, background check, including criminal history, personal references, employment and education verification
- Holds an Alcohol and Other Drug (AOD) Counseling Certification from an approved Certifying Organization (CCAPP, CADTP or CADEE)
- Minimum 1-year general experience in case management
- Knowledge of the disease of alcoholism/drug abuse
- Understanding and experience working with recovering individuals and their impacted family members
- Understand and apply the principles of client-centered, strength-based practice (including the principles of the recovery model and trauma-informed practice)
- Basic Microsoft operating system skills: Outlook, Office, Word, Excel, PowerPoint.
- Familiar with Electronic Health Records (EHR)

- Able to de-escalate threatening/volatile situations with clients, or in the workplace
- Bachelor’s Degree in relevant discipline
- 2 years’ experience working with target population
- Knowledge of/experience with substance abuse services in L.A. County
- ASAM I & II Modules completed
