About Us:

Eight million Americans are one paycheck away from becoming homeless. In Washtenaw County, 5000 people are experiencing homelessness. The Shelter Association of Washtenaw County (SAWC) was established in 1982 and is the primary provider of services and emergency shelter for individuals experiencing homelessness in Washtenaw County. SAWC is housed at the Robert J. Delonis Center in downtown Ann Arbor. SAWC works with a diverse range of people struggling with homelessness, including first-time homelessness, veterans, domestic violence survivors, and people battling substance abuse and/or mental health issues. Services are offered through our core programs: Residential Emergency Shelter, Non-Residential, Shelter Diversion, Critical-Time Intervention, and Warming Shelter. The SAWC serves nearly 1500 people annually.

Position Profile: The Recuperative Care Coordinator (RCC) is responsible for coordination and outreach with local health providers to accept clients into the SAWC’s medical respite program. Once admitted, the Coordinator creates individual service plans that outlines housing readiness case management provision. The Recuperative Care Coordinator also provides referrals for basic needs and prepares a plan with the client to identify safe, affordable housing as soon as possible. They work extensively with community resources and act as a representative of SAWC while liaising with community partners and health agencies. The Recuperative Care Coordinator is expected to contribute to the efforts to secure long term funding sources for the longevity and growth of the Recuperative Care Program. Lastly, this role serves as a task supervisor to two Operation Specialists within the program to ensure the client’s basic needs are met.

The position is a salaried, exempt position with an annual salary range between $52,000-58,000. This role is represented by our Union, the Delonis Workers United of Ypsilanti Industrial Workers of the World.

The position is funded through local funding and SAWC is interested in securing additional funding to sustain the RCC position for more than three years.

Recuperative Care Coordinator Responsibilities:

• Support recuperative care program operations team members as needed
• Aid shelter staff in dealing with immediate crisis situations for at Recuperative Care site
• Act as a liaison for organizations in the community including hospital and health systems and Packard Health
• Oversee the successful delivery of program outputs; monitor and evaluate program effectiveness
• Be on-call for recuperative care program as required and available for site and over the phone needs throughout evenings and weekends
• Coordinate, review, and accept client referrals from hospital discharge staff
• Act as a point of contact for referring partner concerns and questions

• Educate new partners and hospital discharge staff about RC referral process
• Outreach to health systems when needing to further assess referral appropriateness
• Attend Program Taskforce and care coordination meetings
• Coordinate with nursing staff as needed
• Coordination of discharge prevention plan for behavioral related reasons with support of Program Director
• Monitor medication administration procedures and ensure all staff are trained and follow medication log protocol

Schedule: Monday- Friday 8:30 to 5pm, occasionally 10:30 to 7 and weekends as needed

Qualifications:

• Bachelor’s Degree, professional certification, or alternative equivalent experience may be considered. SAWC greatly values lived experiences and considers it during the hiring process.
• Knowledge of mental health and/or experiences is recommended for this profession.
• Knowledge of housing, mental health, chemical dependency, and poverty issues helpful. Experience in social work, social services, or human services is helpful.
• Have excellent verbal and written communication skills.
• Willingness to adhere to the NASW Code of Ethics.
• Exhibit a commitment to diversity and issues related to poverty.
• Be culturally competent, including working with those who have serious mental illnesses, who are suffering from chemical dependency, or who have varying gender identifications or sexual preferences.
• Skillset in multitasking.
• Computer skills with proficiency in Microsoft Office and use of the Internet in locating resources.
• Have reliable transportation to complete apartment inspections and landlord engagement

• Having strong diagnostic skills especially related to mental health and chemical dependency issues is preferred
• Be mindful of personal boundaries, ethical obligations and the primacy of client needs
• Have a commitment to economic justice, civil and human rights issues
• Be mature in nature and professional in action
• Be reliable and willing to work required shifts, including holidays and weekends.
• Bi-Lingual or American Sign Language ability a plus.
