Job Description

ALM Hopewell Center provides comprehensive mental health services to children and adults in the northern and central parts of the City of St. Louis and North St. Louis City. A professional mental health staff serves a growing caseload of 2,800 patients, of which greater than ninety-five percent are chronically mentally ill and suffering from major psychiatric disorders. Hopewell Center operates a Psychosocial Rehabilitation Center and a Comprehensive Psychiatric Rehabilitation Center for adults and youth.

The candidate will assists patients and their families with personal and environmental difficulties through assessment and individualized intervention.


Essential Duties and Responsibilities:


Works in close collaboration with physicians and other health care personnel in patient evaluation and treatment to further their understanding of significant social and emotional factors underlying patient's health problem. Helps patient and family through individual or group conferences to understand, accept, and follow medical recommendations. Provides service planned to restore patient to optimum social and health adjustment within patient's capacity via social work intervention and/or Mental Health counseling. Utilizes community resources to assist patient to resume life in community or to learn to live within limits of disability. Prepares patient histories, service plans, and reports. Participates in planning for improving health services by interpreting social factors pertinent to development of program/direct patient care. Identifies patients who are socially high risk, assesses psychosocial needs and develops a plan and implements. Provides needed information and referrals to patients and their families in an appropriate and timely manner. Establishes and maintains an effective working relationship with community service organizations and public agencies to ensure maximum access to information and services. Education and/or Experience:


Provisionally Licensed Professional Counselor or Provisionally Licensed Clinical Social Work, currently under supervision (Fully Licensed Professional preferred). Master's Degree from an accredited school in Social Work, Psychology or Professional Counseling. Training and experience in at least one Evidence Based Practice. Twoyears of clinical experience. Must have excellent verbal, written and communication skills.
